Diabetic assessment form

    Hi I am trying to put together a Diabetic assessment form that meets the standards required by AHPRA. I have got all the basics such as pulses, skin colour, texture and temperature, sensation (monofiliment, tunifork), what else would you suggest to include in the form to provide a more accurate outcome??
