    @Kevin Kirby : I presently have a very interesting case I would like your opinion. The pt. is a 24 yr. old former college wrestler. He displays hypermobility throughout both feet upon ROM. Currently he has:
    -failed Brostrom-Gould
    -torn ATF and AITF ligaments
    -anterior ankle impingement
    -short first metatarsal with impaction OCD which the bone marrow edema extends 8mm into proximal metatarsal. OCD measures 4.2mm
    -1st metatarsal elevatus of 4mm
    -torn flexor plate 1st MPJ
    -small dorsal osteophyte 1st metatarsal head
    -functional hallux limitus
    -upon palpation and plantarflecting the 1st metatarsal pt. experiences no pain upon ROM. There is severe pain otherwise.
    -gait analysis reveals Achilles' tendons vertical
    -gastroc equinus
    This is all left foot. Do you have any suggestions?

    The item not mentioned is the length of time patients were followed. One important issue with surgical treatments of hallux rigidus is what can be a significant difference between short term and long term results.

    Like many studies, the stated results are based on relatively short term outcomes. A complete first MTP joint arthrodesis means no joint pain as the joint no longer exists.

    What are the long term biomechanical effects of altered first ray function?

    If the study only asks about pain, and arthrodesis removes all that pain, then it will be deemed a success. Very few papers seem to look at the long term biomechanical effects so it is a very good question.

    Does anyone actually ask the patient what is important to them? Are patients informed that other joints will be affected by the fusion of a joint and do they actually understand the implications of that? Do the follow up questions include finding out if the patient now has pain in their knees or hips or back?

    I would really like to see the James Lind Alliance get involved in podiatric conditions so that we can have better research instead of the same old 'no pain after arthrodesis so it must be good' type stuff.
