Achilles tendon rupture

    "The transfer of Flexor Hallucis Longus (FHL) in the management of chronic Achilles tendon ruptures has demonstrated good functional outcome, but an extensive surgical field at a vulnerable location may lead to increased risk for soft tissue problems. The arthroscopic FHL transfer may reduce the risk for soft tissue problems. Functional outcome parameters are investigated, wound/soft tissue complications registered."


    Seems pretty ambitious to think the FHL tendon and muscle that has considerably less physciologic cross sectional area compared to the gastroc or soleus could create anywhere near the plantar flexion power of the gastroc and soleus. Additionally if they transfer the FHL to the Achlles insertion, it will have a smaller lever arm than the Gastroc and soleus. One more thing, the FHL tendon is a bipennaate muscle and does not have the ability to shorten for a significant length. They might be transferring the tendon to a location where the muscle doesn't have excursion (max stretched length - max contraction length) to move the ankle joint through its full range of motion. Knowing that, I would not consent to having that procedure done on me. I also would advise anyone considering the procedure against doing it. It does not seem that what you would gain, would be worth what you would lose. (That is if the surgery went perfectly as intended.) We can only hope they publish the research even it doesn't come out as good as the authors hoped.
