    Piezogenic Pedal Papules?

    Hi everyone!

    I am a fourth year podiatry student at Glasgow Caledonian and this is my first post. Today in clinic I had a patient with piezogenic pedal papules. The only reason I know the patient had this was because he told me. I have never heard of them before, let alone seen them! I have came home and tried to read up on them, however my resources seem a bit vague? Can anyone please tell me anymore about them, i.e what causes them, why they occur and any treatment methods? Any information will be extremely grateful!

    75% of the population have them!, but only a very few are ever painful.

    Hello,

    I have a female patient (mid 30s) with bilateral painful piezogenic pedal papules (around all borders of both heels and extending anteriorly to the level of the navicular medially). She is moderately overweight and has a job that involves constant standing, which is not modifiable. She wears well-fitting training shoes. Oral analgesia has been unsuccessful.

    Unfortunately, there seems to be little in the literature to guide the practical, clinical management of painful papules in adults. Deep heel cups and betamethasone/bupivacaine injections seem to be the two options available.

    Does anyone have any clinical pearls to share regarding different treatment approaches they have tried, and what success has been achieved with these?

    In addition to giving the patient advice about weight reduction (a long term goal for her), my plan A is to get her into orthotics with a deep enough heel cup around the papules to reduce the extent of the herniations on weightbearing.

    I would welcome information about other approaches that have met with success.

    Opinions please , Piezogenic pedal papules.
    I have seen a lady today early 30's, no meds and no other symptoms other than they are sore at times ,she is hypermobile but no complaints regarding that .
    She has lesions that look similar to Piezogenic pedal papules images seen at Google images. However they appear on the plantar mpj areas unilateral rather than around the heel ,where most images show them. I am uploading photo's.I have looked at these lesions under 10x illuminated eschenbach magnifier (which I could not do without),whilst at first glance you consider vp , but looking deeper these do not appear so.
    The only palliative remedy I could think of are the gel insoles that some advertisers push.
    Can any one please give their opinions or experience. Is there anything I should look at or consider

    Classic Piezogenic nodules or papules occur around the heel area. I have had 3 or 4 of these symptomatic over the years, or just bothersome because of the size, that surgery was indicated. Upon removal they appear as normal adipose tissue.

    I do not relate these to plantar forefoot lesions.

    Your pics are somewhat small, but my educated DDx (based only on the pictures) : early verrucae; intradermal fibromas; fibrosis from previous treatment; nonspecific dermatisis.

    Hi had 1 case in 30 years gave her steriod injections and surgical hosiery problem resolved. ps they were in both heels plantar aspect.
