Comparison of effectiveness of electrocautery and phenol application in partial matricectomy after partial nail extraction in the treatment of ingrown nails

Ingrowing nail also known as onychocryptosis is a common health problem. This disease mostly affects young people, often carrying a considerable amount of socio-economic implications. It’s foot problem that usually manifests as inflammation of tissue along the side of a toenail.

The aim of the study was to asses and to compare effectiveness of electrocautery and phenol application in partial matrixectomy after partial nail extraction in the treatment of ingrown toenails.

Material and methods. The group of 60 patients with ingrowing toenail which was randomized into two groups underwent partial matrixectomy in surgical outpatient clinic between 2009-2013. This group of patients was under surgical observation for 100 days in outpatient clinic.

Results. In all operated patients we obtained surgical success however we had 13 recurrences during the follow up period, 5 in the phenolization group and 8 in the electrocoagulation group.

Conclusions. There was statistically significant difference between these two techniques, which indicated that matrix phenolization is connected with shortened healing time vs the matrix electrocoagulation

THE COMPARISON OF ELECTROCAUTERY AND CURETTAGE OF THE NAILBED FOR THE TREATMENT OF INGROWN TOENAIL

Background and aim: The ingrown toenail is a condition of the active young population that often seriously impairs the patient’s comfort, causing distress to walk, which is seen in the second and the third decades. We aimed to compare two different treatment techniques for that disease.

Methods: A total of 80 patients who underwent surgery, using the Winograd technique, due to an ingrown toenail were included. The mean age of the patients was 29 (21-44) years. There were 32 female (40.0%), and 48 male (60.0%) patients. The patients were divided into two groups: Group 1 (n = 40) consisted of patients in whom electrocautery was applied during the surgery, whereas in Group II (n = 40) curettage was done. Recurrence, and infection rates were compared.

Results: The statistical analysis revealed no significant difference between the two groups in terms of recurrence, and infection rates (p> 0.005).

Conclusion: Our results showed that there is no superiority of one of these methods to the other in terms of the recurrence, and infection rates.

The aim of this study was to compare the result of electrocauterization and curettage, which can be done with basic instruments. Patients with ingrown nail were randomized to 2 groups. In the first group, nail matrix was removed by curettage, and the second group, nail matrix was removed by electrocautery. A total of 61 patients were enrolled; 32 patients were operated by curettage, and 29 patients were operated by electrocautery. Wound infections, as early complication, were found in 15.6% (5/32) of the curettage group, 10.3% (3/29) of the electrocautery group patients each (P = .710). Nonrecurrence was observed in 93.8% (30/32) and 86.2% (25/29) of the curettage and electrocautery groups, respectively, (lower limit of 1-sided 90% confidence interval = −2.3% > −15% [noninferiority margin]). To remove nail matrix, the curettage is effective as well as the electrocauterization. Further study is required to determine the differences between the procedures.

Introduction: There are different cures protocols to manage total chemical matricectomies.
Recently, the controlled flux electrolyzed acid solution has been used to treat burns, so, the aim of
the study was to compare the total healing time and bacterial load in two types of cures.
Patients and methods: In a sample of 28 chemical total matricectomies, in the first postoperative
visit (48 h), two groups were made at random: group 1, applying of controlled flux electrolyzed
acid solution (maintaining ever wet the bandage); group 2 (iodate povidone in gel) were made. A
count of bacterial colony forming units was taken at this postoperative visit and three days later.
Also, the evolution of both groups was followed every three days, to determine the total healing
time.
Results: Mean days of discharge were lower (29.2 ± 3.2 days) in the Group 1 than in group 2 (34.3
± 4.1), p = 0.023. The bacterial load at the 48 hours first visit was not different between groups, (p
> 0.05). At the second culture (5 days after the surgery), the bacterial load at Group 1 was 0.42 ±
0.38 × 104 and in Group 2, 0.63 ± 0.48 × 104 with a statistical difference (p = 0.012) between both.
Conclusion: In total chemical matricectomies, the protocol of cures with the controlled flux
electrolyzed acid solution (SAEFC) provides a 5 days reduction in the time of discharge. Also, a
lower bacterial load was observed in this cures protocol, which could help to prevent infections in
toenails surgery

Effect of Phenol Application Time in the Treatment of Onychocryptosis: A Randomized Double-Blind Clinical Trial

Background: In the treatment of Onychocryptosis, chemical matricectomy with 88% phenol solution is one of the most common surgical procedures due to a recurrence rate of less than 5%, but it may produce a delay in healing time. The objective was to compare the healing time between phenol applications of 30 or 60 s.

Methods: A comparative, prospective, parallel, randomized, and blinded clinical trial was registered with the European Clinical Trials Database. Twenty-seven patients (54 feet) with 108 affected nail folds were randomized and treated with chemical matricectomy with phenol. Each hallux was randomly assigned to one of two groups (60 vs. 30 s phenolization). Each patient and one investigator were blinded to the phenol application time in each foot. The outcome measurements were healing time, recurrence, pain, post-surgical bleeding, inflammation, and infection rate.

Results: The 30 s application presents a shorter healing time (14.93 ± 2.81 days vs. 22.07 ± 3.16 days; p < 0.001) with a similar recurrence rate (p = 0.99). Post-operatory bleeding, pain, inflammation, and the infection rate did not show significant differences (p > 0.05).

Conclusions: The 30 s phenol application time offers a shorter healing time than 60 s without affecting the effectiveness of the procedure, showing the same rate of complications.

The onychocryptosis, also known as ingrown toe-nails (IGTN), is a painful, common disorder which is less common in children than in adults. The purpose of the present study was to focus on the effect of electrocautery matricectomy on recurrence rate and clinical outcomes in different age groups. We performed a retrospective assessment of 189 consecutive IGTN surgeries. Electrocautery matricectomy was performed in 68 (49.2%) of 138 (73%) adults, 25 (49%) of 51 (27%) adolescents. Recurrence was observed in 11 (21.5%) adolescent patients, while recurrence was observed in 12 (8.6%) adult patients. Recurrence was observed in 9 (9.6%) of 93 patients in whom cautery was used, while 14 (14.5%) recurrences were observed in 96 patients who did not use cautery. When the adolescent patient group was evaluated separately, recurrence was observed in 2 (8%) of 25 patients in the cautery group, while recurrence was observed in 9 (34.6%) of 26 patients in the other group. EM addition to the wedge excision does not affect the results in adult patients, but it significantly reduces recurrence in adolescent patients. Especially in younger patients, it is recommended to complete the matricectomy with electrocoagulation.

Introduction: Ingrown toenails are one of the common nail problems generally occurs in the big toe area and most often occurs during productive ages. Treatment of ingrowing toenail can be chosen based on the clinical condition of the patient. Nonoperative therapy is not effective in advanced cases in terms of the severity. Improper handling will cause recurrence in this case. Case Presentation: Six of 8 patients had the ingrown toenail recurrence previously treated using the nail avulsion (onychectomy) without matricectomy. One patient was reported to have hemophilia as comorbidity. The other one had pincer nail associated with inflammatory and degenerative processes. Most patients were at modified Mozena grades III and IV ingrown toenail. Post therapy evaluation revealed no recurrence in all patients. Conclusions: Combination of wedge resection with matricectomy using electrocautery can be applied as therapy for recurrent ingrown toenail.
