Burden of Diabetic Foot Patients’ Caregivers and Affecting Factors: A Cross-Sectional Study

With the increase in the diabetic foot patients in recent decades, the caregivers of diabetic foot patients increase too. Most of these caregivers are informal caregivers. However, the studies examining the burden of the caregivers and affecting factors are limited. This study was conducted to determine the burden of the caregivers of diabetic foot patients and affecting factors. This cross-sectional study was conducted between the January and October 2020 in a diabetic foot council of a university hospital. Zarit Caregiver Burden Scale and a participant identification form were used for data collection. Most of the caregivers were female (75.2%) and the mean age was 51.27 ± 11.48 years. The burden of the caregivers was at moderate level in the current study. Factors affecting the caregivers’ burden were caregivers’ age, patients’ family structure, caregivers’ education level, caregivers’ income level, hours per week spending for the care of the patients, and lack of choice.

Prevention and Management of Diabetes-Related Foot Ulcers through Informal Caregiver Involvement: A Systematic Review

Background: The literature remains unclear whether involving informal caregivers in diabetes self-care could lead to improved diabetic foot outcomes for persons at risk and/or with foot ulcer. In this review, we synthesized evidence of the impact of interventions involving informal caregivers in the prevention and/or management of diabetes-related foot ulcers.

Methods: A systematic review based on PRISMA, and Synthesis Without Meta-analysis (SWiM) guidelines was conducted. MEDLINE (Ovid), Embase (Ovid), PsycINFO, CINAHL, and Cochrane Central Register of Controlled Trial of the Cochrane Library databases were searched from inception to February 2021. The following MESH terms were used: diabetic foot, foot ulcer, foot disease, diabetes mellitus, caregiver, family caregiver ,and family. Experimental studies involving persons with diabetes, with or at risk of foot ulcers and their caregivers were included. Data were extracted from included studies and narrative synthesis of findings undertaken.

Results: Following the search of databases, 9275 articles were screened and 10 met the inclusion criteria. Studies were RCTs (n = 5), non-RCTs (n = 1), and prepoststudies (n = 4). Informal caregivers through the intervention programmes were engaged in diverse roles that resulted in improved foot ulcer prevention and/or management outcomes such as improved foot care behaviors, increased diabetes knowledge, decreased HbA1c (mmol/mol or %), improved wound healing, and decreased limb amputations rates. Engaging both caregivers and the person with diabetes in education and hands-on skills training on wound care and foot checks were distinctive characteristics of interventions that consistently produced improved foot self-care behavior and clinically significant improvement in wound healing.

Conclusion: Informal caregivers play diverse and significant roles that seem to strengthen interventions and resulted in improved diabetes-related foot ulcer prevention and/or management outcomes. However, there are multiple intervention types and delivery strategies, and these may need to be considered by researchers and practitioners when planning programs for diabetes-related foot ulcers

Background: Diabetic foot ulcer remains the leading cause of non-traumatic amputations globally. Informal caregivers (persons providing unpaid services at home) can play significant role in patients’ disease management and involving them in self-care interventions could lead to improved diabetic foot outcomes.
Aim: (1) To determine the effects of interventions involving informal caregivers in the prevention and/or management of diabetic foot ulcers in adults and (2) To understand if combinations of intervention components were associated with particular foot ulcer clinical outcomes.
Method: A systematic review based on Preferred Reporting Items for Systematic reviews and Meta-Analysis (PRISMA) and Synthesis Without Meta-analysis (SWiM) guidelines was conducted. The protocol was duly registered with the International Prospective Register of Systematic Reviews, PROSPERO, with registration number CRD4202123176. MEDLINE (Ovid), Embase (Ovid), PsycINFO, Cumulative Index of Nursing and Allied Health Literature (CINAHL) and Cochrane Central Register of Controlled Trial of the Cochrane Library databases were searched from inception to 28 February 2021. In addition to keywords optimised for each database, the following MESH terms were used, diabetic foot, foot ulcer, foot disease, diabetes mellitus, caregiver, family caregiver and family. Experimental studies involving persons with diabetes, with or at risk of foot ulcers and their caregivers (dyad) were included. Data was extracted from included studies and narrative synthesis of findings undertaken.
Results: Following the search of databases, 9275 articles were screened and 10 met the inclusion criteria. The review identified that the involvement of informal caregivers in interventions resulted in improved foot care practices, decrease foot ulcer incidence, improved wound healing and decrease HbA1c. Even though not all studies were with controlled groups, the included five Randomised Controlled Trials all indicated that these outcomes were better in the caregiver involved groups compared to no caregiver involvement. Informal caregivers were involved in the programmes as supportive persons, assisting in wound care, daily inspection of feet, using monofilament to check feet sensation and diet/meal planning.
Engaging both caregivers and the person with diabetes in hands-on skills training on wound and/or foot care combined with education were peculiar components of interventions that consistently produced clinically and statistically significant improvement in wound healing and reduction in incidence of foot ulcers. Other key elements of these interventions were follow up telephone calls reminding participants and caregivers to perform foot care and report any foot problems to the clinic after the face-to-face interactions. However, addressing negative and inaccurate beliefs about diabetes through only education over the telephone did not result in significant improvement in foot care and HbA1c.
Discussion: The involvement of informal caregivers strengthened the interventions and resulted in improved foot care practices and favourable diabetic foot clinical outcomes. However, the effects of these interventions differed based on the combination of different intervention elements, and these may need to be considered by researchers and practitioners when planning foot care programmes. Future research may need to assess the characteristics of caregivers and how their involvement may impact on their own health.

Stress Reduction Interventions with Patients with
Chronic Diabetic Foot Ulcers: A Qualitative Study

Background
The present study aimed to assess the perceptions of patients with chronic diabetic foot ulcers (DFUs) and
their informal caregivers regarding the impact of two stress reduction interventions, such as a progressive
muscle relaxation and hypnosis, on patients’ DFU and psychological wellbeing.
Methods
This multicenter study used a qualitative exploratory design and included individual interviews with eight
patients with chronic DFUs and six family caregivers, using a semi-structured interview guide. Transcripts
analysis employed thematic content analysis.
Results
Four key themes common to patients and their caregivers were found: 1) perspectives regarding the
intervention; 2) intervention effectiveness; 3) the role of psychology on the DFU treatment; and 4) emotions
and consequences associated with the DFU. Although themes were common to both treatment groups, subthemes from the last two themes differed for patients that received muscle relaxation versus those who
received hypnosis. One additional theme emerged from the caregiver’s interviews: 5) promotion of self care
behaviors.
Conclusion
According to patients and caregivers, overall the two stress reduction interventions were beneficial for the
DFU healing progression and emotional wellbeing. The hypnosis group also reported lasting effects.
Participants suggested that psychological interventions such as stress reduction interventions could be
included in the DFU standard treatment as an adjuvant to the clinical protocol for DFU treatment, preferably
offered early on, when the patient begins treatment at the diabetic foot consultation.

Changes in lifestyle can cause degenerative diseases such as Diabetes Mellitus (DM). The prevalence and incidence of this disease has increased drastically in industrialized and developing countries, including Indonesia. WHO predicts that DM data will increase to 300 million in the next 25 years. The International Diabetes Federation (IDF) estimates that in 2020 there will be 178 million DM sufferers in Indonesia aged over 20 years and assuming a DM prevalence of 4.6%, there will be 8.2 million DM patients. Currently, the prevalence of DM based on the International Diabetes Federation (IDF) in 2019, Indonesia is ranked 6th in the world with 10 million people. Based on the 2007 Riskesdas results, it is estimated that 463 million people suffer from diabetes currently and it is projected to reach 578 million in 2030. and 700 million in 2045. This study was to determine the relationship between family support and the degree of Diabetic Foot Ulcers (DFU). The design of this research is an analytical observational study with a cross-sectional approach design. The subjects of this study were DM sufferers with diabetic foot ulcers who underwent treatment at the Independent Wound Nursing Practice in Bone Regency. The sampling technique uses the Total Sampling method. After carrying out the Pearson correlation test, test results were obtained with a p value = 0.000 and an r value = 0.648** so it can be concluded that there is a very strong correlation between family support and the degree of diabetic foot ulcers. So it can be concluded that the higher the family support for patients with diabetic foot ulcers, the lower the degree of injury. The results of this study can be concluded that there is a strong relationship between family support and the degree of injury in Diabetic Foot Ulcer (DFU) patients so that caring for families with diabetic foot ulcers will provide good encouragement for diabetic foot ulcer patients so they can heal and follow the treatment program. the good one

Effectiveness of continuous home wound care on patients with diabetic foot ulcers

Aims: To explore the effectiveness of continuous home wound care on patients with diabetic foot ulcers (DFUs).

Design: A non-randomized parallel controlled non-inferiority trial.

Methods: Patients with Wagner grade I-III DFUs hospitalized in two distant campuses of the same hospital were included. All patients received infection treatment and wound bed preparation during hospitalization; after discharge, patients in one of the campuses received routine outpatient wound care, and those treated in the other received continuous home wound care. The per-protocol analysis was performed to compare ulcer healing indicators, knowledge, health belief, self-management behaviour and medical expenses of the two groups.

Results: Between October 2021 and December 2022, 116 patients were enrolled in the study; 107 completed. The home care was not inferior in terms of ulcer healing rate and demonstrated significant enhancements in the understanding of warning signs, health belief and self-management behaviour. Additionally, the home care saved 220.38 yuan (24.32 UK pounds) in direct medical expenses for each additional one square centimetre of ulcer healing.

Conclusion: The continuous home wound care enhanced self-management behaviour of the patients and saved their medical expenses while not compromising ulcer healing.

Impact: This is to date the first study to conduct continuous home wound care practice for patients with DFUs and confirmed its safety and non-inferiority in ulcer healing, and supported its superiority in improving self-management behaviour and saving medical expenses.

Reporting method: We have adhered to the transparent reporting of evaluations with nonrandomized designs statements and the corresponding checklist was followed.

Patient or public contribution: The patients and their primary caregivers were involved in intervention design, we received input from them about the factors that facilitate and hinder patient self-management behaviours to develop intervention strategies.
