Cortisone Shot, can shot wear off quickly. How long should I give it?

I had severe pain in my right foot and extending into my right leg, hip as well for almost three weeks. Went to Er at two week duration as I also had dangerous increase in blood pressure because of pain. Blood clot was ruled out, ( great!) but Xray revealed loose screw from prior fusion of first Metatarsal which had severe arthritis, also fractured and dislocated after fusion in 2012. (fracture healed.)
Saw my Ortho dr. on Feb. 12, he was not concerned over "loose screw" said it should not...

Cortisone Shot, can shot wear off quickly. How long should I give it?
