    Highlights


    • Limited knowledge exists on interventions targeting functional adaptability of toe flexor muscles in diabetes.

    • Strength training inducing MPJ plantarflexion moments can counteract muscle loss in diabetic feet.

    • Despite atrophied muscles and severe weakness, toe flexor strength increased continuously and was linked to MRI-detected muscle gain.

    • Strength gains were associated with reduced plantar peak pressure at common ulceration sites.

    • We recommend incorporating targeted strength training of intrinsic foot muscles into diabetic care at an early stage.
