Gathering the evidence

    Sorry for cross posting (the following is in my introduction)!

    I am working on behalf of SCP with the DH's QIPP (Quality, Innovation, Prevention and Productivity) Advisory Group which is about delivering services differently. As part of this I have been gathering examples in podiatry of innovative practice and service delivery and great news we will have 3 podiatry examples on the NHS Evidence website from 2 Dec.

    More is wanted for phase 2 and for a report SCP is writing so if you have any examples of innovative practice within your area of work please let me know. My email address is liz@scpod.org

    Oh, dear God.
    I'm sorry, but this post just caused my heart to sink into my boots. Maybe it's because its Monday morning....maybe because I've worked in the NHS for what feels like a lifetime and I suspect all the lovely time and money spent in compiling these examples won't lead to any actual improvements in services across the country.
    Still, maybe I'll be proved wrong and won't actively snort in derision at the phrase "great news""

    I worked in the NHS for eons and am probably as cynical as you. The difference this time (or so it seems) is the view of the DH that whilst money needs to be saved service quality must be maintained.

    The more examples we can give of the cost benefits of the innovative podiatry services provided the better. I am looking for examples that reflect the 10 high Impact areas proposed by the Faculty of Management:

    • Podiatric Surgery
    • POMs
    • Acute Foot, prevention of amputation
    • Facilitate Independent Living a) general fitness b) prevent going to hospital c) help to leave hospital quicker
    • Advanced Technology
    • Maintaining Mobility
    • MKS/Triage
    • Healthy Footwear
    • Prevention Work
    • Education /Health Promotion
    • NICE Guideline 10

    Aren't the 2 mutually exclusive though?

    We are going through a service review/redesign/whatever you want to call it and have been advised the dept needs to cut 10% of it's outlay.....or else.

    I'll shut up now as I might get into bother.

    Which is why SCP are getting involved with QIPP and are developing an evidence based resource pack for managers giving the cost benefit of podiatry. L

    As SCP represents suppliers of footcare from private sector, industrial sector, education and NHS is the Faculty of Management representing all suppliers in its innovative service delivery design?

    SCP is ideally placed to describe (and its members deliver) a diverse innovative service which is cost saving, productive and high quality.

    Therefore can members from the private sector add any innovative ideas for the QUIPP advisory group. Cross sector provision seems an ideal solution.

    Good luck with your project, however if the SCP thinks it can table an innovative service delivery package for foot health services within the NHS - one which utilises the resources of all its members, it is sadly delusional - and a decade too late! More likely, any research project the SCP commissions will seek to promote the status quo - service redesign that contracts the service capacity in line with current and future funding targets whilst claiming justification on the basis of spurious "risk" categorisation. If the SCP were truly serious about its members' interests and access to quality foot health services it would follow the dental example and withdraw wholesale from the NHS then seek to negotiate properly funded care through a devolved practice scheme with a new administration following the next general election. But of course it wont, not least because the status quo directly benefits many of the decision makers at Fellmongers - think; your own Employment Relations Dept, colleagues within the FoM, colleagues on Council who have duality of roles with the NHS - for example Workforce Planning. I look forward to reading your respose to George's points made in the previous post.

    Like many other aspects of public service, the NHS stumbles from one crisis to another - usually making the same mistakes as it did in the last round and some new ones too - like Agenda for Change, NHS IT system (which is expected to be dropped this Wednesday after costing £billions!). The government and the Department of Health has its own agenda. The Society has its agenda too. Neither of them holds any attraction to the vast majority of SCP members and neither is beneficial to any member of the public with foot health problems.

    Hi George

    This project is about innovative practice within the NHS that improves both quality and productivity.

    My original email outlining the project and inviting examples was sent to all SCP Faculties for information and/or action. All podiatry examples, irrespective of their source, would have been considered as long as they met the criteria but I have to say all submissions published on the website are from the NHS. None are from the private sector or industry.

    Thank you for your response.

    As SCP does not have a faculty representing private practice I assume you have made contact with the Committee of Private Practice?

    This is an ideal time for SCP to be leading the way on providing a cross sector quality and innovative service. The private practice survey certainly shows a willingness of the private sector to be involved in cost effective service delivery. It is sad the preference seems to be an alliance with non podiatrists in the not for profit sector. But that is another subject.

    Thank you for supplying the link to the NHS evidence website. No wonder the orthopedic surgeons are up in arms. The SCP has provided good evidence of quality and cost effective foot surgery and a very strong argument for moving foot surgery into primary care.

    You may recall that last October SCP submitted evidence to QIPP which resulted in 3 areas of best practice in podiatry being published on the NHS Evidence website. We have now been invited to submit new evidence to the next round and I have been asked to co-ordinate our response. If you have any examples that you would like to submit, please complete the template available from http://www.evidence.nhs.uk/aboutus/Pages/AboutQIPP.aspx.

    This closing date is 18 June 2010.
