BACKGROUND:
Inherited epidermolysis bullosa is a rare disease characterised by mechanical fragility of the skin when under insignificant stress. The main consequences of epidermolysis bullosa, mainly the dystrophic type, despite pseudosyndactyly, are joint contractures and deformities in hands and feet. In this study, we describe our experience treating patients suffering from epidermolysis bullosa, as far as feet deformities are concerned.
METHODS:
This is a retrospective analysis of a consecutive series of patients presenting feet deformities related to epidermolysis bullosa. Extension contractures of the toes, equinus and cavus deformities were treated with soft tissues surgery.
RESULTS:
Thirteen surgical procedures were done in six patients with feet deformities caused by epidermolysis bullosa. Of the feet operated 85.7 % extension contracture of the toes was asymptomatic at follow-up. However, 42.9 % developed hammertoe deformities. There were no recurrence or complications for other deformities. Subjectively, all patients declared themselves very satisfied with the results.
CONCLUSION:
Foot deformities must be treated as early as possible, due to progressive disability for walking and pain symptoms. We considered that, despite long term complications, treatment was adequate and we recommend it. Level of Evidence Level IV.

An awkward condition to work with and so many different levels of severity.

l had an enquiry in the last week from a potential client in Germany that is reporting hot weather causing issue.

l have never had any of my EB clients report this other than they dont wear there protective footwear in the warmer weather so there are more knock injuries.

Has anyone else had EB clients report hot weather directly causing issue?

Foot care in Epidermolysis bullosa: Evidence-based Guideline.

To provide service providers and users with an evidence-based set of current best practice guidelines for people and their families and carers, living with Epidermolysis bullosa (EB). A systematic literature review relating to the podiatric care of patients with EB was undertaken. Search terms were used, for which the most recent articles relating to podiatric treatment were identified as early as 1979 to present day, across seven electronic search engines: Medline, Wiley online library, Google Scholar, Athens, Researchgate, Net and pubfacts.com. The Scottish Intercollegiate Guidelines Network (SIGN) methodology was used. The first guideline draft was analysed and discussed by clinical experts, methodologists and patients and their representatives at four panel meetings. The resulting document went through an external review process by a panel of experts, other health care professionals, patient representatives and lay reviewers. The final document will be piloted in three different centres in United Kingdom and Australia. Following an EB community international survey the outcomes indicated six main areas which the community indicated as a priority to foot management. These included blistering and wound management; exploring the most suitable footwear and hosiery for EB; management of dystrophic nails; hyperkeratosis (callus); maintaining mobility; and fusion of toes (pseudosyndactyly). Evidence here is limited but several interventions currently practised by podiatrists show positive outcomes.

Epidermolysis bullosa (EB) is a genetic skin fragility disorder which, in the majority of cases, causes pain and functional impairment because of involvement of the skin of the feet. We wish to highlight the importance and need for specialized podiatry services for individuals with this condition.

Clinically, EB is characterized by skin blistering secondary to minor mechanical trauma or friction. There are many different subtypes, with a broad spectrum of severity and variable extracutaneous features.1 The most common form of EB is EB simplex (EBS) localized where blistering is predominantly confined to feet and hands; management involves avoidance of trauma by means of comfortable, soft footwear; avoidance of heat; puncturing blisters when they arise to prevent expansion; atraumatic dressings; and avoidance or management of infection when it arises.2,3 However, all major forms of EB can affect the feet with blistering, fissuring, hyperkeratosis, altered gait, and nail...

Management of Skin Lesions in Patients with Epidermolysis Bullosa by Topical Treatment: Systematic Review and Meta-Analysis

Epidermolysis bullosa (EB) is the overarching term for a set of rare inherited skin fragility disorders that result from mutations in at least 20 different genes. Currently, there is no cure for any of the EB subtypes associated with various mutations. Existing therapies primarily focus on alleviating pain and promoting early wound healing to prevent potential complications. Consequently, there is an urgent need for innovative therapeutic approaches. The objective of this research was to assess the efficacy of various topical treatments in patients with EB with the goal of achieving wound healing. A secondary objective was to analyse the efficacy of topical treatments for symptom reduction. A literature search was conducted using scientific databases, including The Cochrane Library, Medline (Pubmed), Web of Science, CINHAL, Embase, and Scopus. The protocol review was registered in PROSPERO (ID: 418790), and inclusion and exclusion criteria were applied, resulting in the selection of 23 articles. Enhanced healing times were observed compared with the control group. No conclusive data have been observed on pain management, infection, pruritus episodes, and cure rates over time. Additionally, evidence indicates significant progress in gene therapies (B-VEC), as well as cell and protein therapies. The dressing group, Oleogel S-10, allantoin and diacerein 1%, were the most represented, followed by fibroblast utilisation. In addition, emerging treatments that improve the patient's innate immunity, such as calcipotriol, are gaining attention. However, more trials are needed to reduce the prevalence of blistering and improve the quality of life of individuals with epidermolysis bullosa.
