Nail surgery forms

    Hi all!

    Not sure how this site really works, thus probably putting this request in wrong section. No doubt some of you will let me know if i have lol!!

    Myself and a colleague are revising our departmental nail surgery ass and procedure forms. The purpose of this posting is to see if any of you would be kind enough as to share or perhaps forward what you currently use in your practice to enable us to do this. Internet searching has not been very productive.

    Hi Louise,

    I'd be happy to email you the forms I use in my practice if you pm me your email. Also if you are a member of a professional body eg. Society of Chiropodists & Podiatrsts they have forms available to download from the members section.

    Hmmmmmmmmm. I can't say that we've ever used ass forms.

    That is because these are very special British forms only authorised for use in England Steve.

    TUSHIE Forms!

    I get it now, our equivalent to the ARSE Forms.

    I had a bunch of these ass forms but they just sat around, getting old, some of them had cracks in them.

    Some were printed wrong, sort of ass backwards
    some were on oversized paper.........wide ass.
    My secretary hated to fill them out, sort of a lazy ass, you know.

    Stop taking the micky out of my posting Steve!! It's not my fault my ass is inadequate! It is in the interest of public safety i get my ass fixed!!!
