Effects of Cigarette Smoking on Elastographic Strain Ratio Measurements of Patellar and Achilles Tendons

Objectives—The aim of this study was to explore the sonographic and elastographic properties of patellar and Achilles tendons in smoking and nonsmoking otherwise healthy adults.

Methods—We conducted a level 3 case-control analytical study. Smoking and nonsmoking volunteers (>18 years) without musculoskeletal system disorders were included in the study. Demographic characteristics and smoking habits (pack-years) were recorded. Proximal, middle, and distal third thicknesses of the patellar and Achilles tendons were measured by B-mode sonography. Strain ratio measurements of the same regions were measured by real-time ultrasound elastography.

Results—A total of 69 participants (57 male and 12 female; mean age ± SD, 35.5 ± 7.8 years) were evaluated in the study. Smoking (n = 35) and nonsmoking (n = 34) groups had no significant differences in terms of age, body mass index, sex, and activity level (all P > .05). Proximal, middle, and distal thirds of the patellar and Achilles tendons were significantly thinner in the smoking group (all P < .05). Furthermore, strain ratio measurements in the same regions were significantly lower in the smoking group (all P< .05). Patellar tendon thicknesses and strain ratios had negative correlations with the smoking amount (all P < .05).

Conclusions—Thickness and strain ratio measurements of patellar and Achilles tendons were reduced (thinner and harder tendons) in smokers. Clinical implications of these morphologic and elastographic changes should be investigated in future studies.

Meta-analysis of Cigarette Smoking and Musculoskeletal Injuries in Military Training.

Purpose: Tobacco use is common among military personnel, as is musculoskeletal injury during training. In a review of the literature on musculoskeletal injuries there was mixed evidence on the role of smoking as a risk factor. The purpose of this study is to review and analyze the literature on the impact of cigarette smoking on lower extremity overuse injuries in military training.

Methods: We performed a literature search on articles published through October 2016. Search terms focused on lower extremity overuse musculoskeletal injuries and cigarette smoking in military populations. We conducted a meta-analysis overall and by sex, including smoking intensity.

Results: We identified 129 potential studies and selected 18 based on quality. The overall rate ratio for smoking was 1.31, 1.31 for men, and 1.23 for women. Overall and for each sex, rate ratios were significantly greater than 1.0 for each intensity level of smoking.

Conclusions: Smoking is a moderate risk factor for musculoskeletal injury, and may account for a meaningful proportion of injuries among men and women due to the high prevalence of smoking and injury in this population. Although enlistees are not allowed to smoke during basic training, their risk of injury remains high, indicating that smokers may remain at increased risk for medium to long term duration.

First 30 Days Postoperative Wound Related Complications in Young Smokers Following Primary Achilles Repair

The Achilles tendon is frequently injured in the young to middle aged population. Previous studies have shown an increased association between cigarette smoking and delayed wound healing among patients with Achilles tendon injury.
This study utilized the American College of Surgeons National Surgical Quality Improvement Project (ACS-NSQIP) database. We included patients between the ages of 15 and 35 years, and who underwent primary Achilles tendon repair between years 2011 and 2020. The procedures with or without graft (CPT 27650 and 27652), patient demographics, and comorbidities were included. Primary outcomes of interest were rates of 30-day readmission, minor complications, outcomes related to wound healing and reoperations within 30 days of index surgery.
A total of 1944 patients met the inclusion criteria for this study. One thousand six hundred and fifty nine patients were nonsmokers, while two hundred and eighty five corresponded to smokers. Logistic regression showed no differences between smokers and non-smokers (reference group) for 30-day readmission, reoperation, and minor complications. However, Black non-Hispanic patients were found to be 0.3 times (95% CI: 0.1, 0.98) as likely to develop minor complications as compared to the White non-Hispanic patients.
Wound-related complications after Achilles tendon repair remain low in younger (15-35 years) patients. When comparing differences in clinical outcomes between non-smokers and smokers, we found no statistically significant difference in this retrospective study.

Examining gender differences in gait parameters between non-smoker and smoker participants

Smoking is one of the predictors of decreased cardiopulmonary endurance. Gait disturbance may be due to many reasons, including cardiovascular endurance. This study aimed to determine differences in gait parameters between non-smoker and smoker participants. A cross-sectional design was employed, involving thirty non-smokers and thirty-seven smokers as participants. Detailed interviews were conducted to gather information on smoking habits, status, and history. Gait parameters were measured using a high-quality 3D accelerometer, 3D gyroscope, and barometric pressure sensors (Physilog4 from GaitUp). Anthropometric characteristics were described, and mean values with standard deviations (SD) were calculated. An independent two-tailed t-test was conducted to compare gait parameters between non-smokers and smokers, with statistical significance set at p<0.05. The analysis revealed significant differences in various gait parameters between non-smokers and smokers. Specifically, significant differences were found in cadence (t=9.95, p=0.001), stride length (t=6.85, p=0.001), stride velocity (t=-6.58, p=0.001), stance (t=2.02, p=0.001), swing (t=3.46, p=0.001), foot flat (t=-8.94, p=0.001), pushing (t=3.53, p=0.001), and double support (t=-13.35, p=0.001). However, no significant difference was found between non-smokers and smokers in the loading phase (t=-1.57, p= 0.121). There were significant differences in general and temporal gait parameters between smokers and non-smokers. Gait parameters provide valuable insights for evaluating functional performance and providing objective and quantitative data to assess gait disorders. Future studies should include longitudinal studies with large sample sizes to explore the effects of potential confounders on gait parameters.
