The objective of this study was to demonstrate the corrective effect of naviculocuneiform (NC) arthrodesis on the medial
longitudinal arch in collapsing foot deformity (CFD) and to establish radiographic criteria for its indication as a stand-
alone or complementary surgical procedure. NC arthrodesis was performed in 17 patients diagnosed with CFD, with
clinical and radiological follow-up conducted to evaluate correction of the medial longitudinal arch. Postoperative
angular measurements were obtained at five weeks under weight-bearing conditions after removal of the short-leg cast.
Statistically significant radiographic improvements (p<0.05) were observed in all parameters except for the calcaneal
pitch angle (p>0.05). The mean preoperative Meary angle was 11.8°, with an average reduction of 10°, resulting in a
postoperative mean of 1.9° (p<0.05). The preoperative talocalcaneal angle in the Saltzman view averaged 6.02°
compared to 2.7° postoperatively (p<0.05). The talo–scaphoid coverage angle decreased from a mean of 34.5°
preoperatively to 22.9° postoperatively (p<0.05), while the talo–scaphoid incongruence angle improved from 39.3° to
24.2° (p<0.05). Although the calcaneal pitch angle improved slightly (19.1° to 21.0°), the change was not statistically
significant (p>0.05). NC arthrodesis proved to be an effective technique for correcting collapsing foot deformity,
achieving significant restoration of medial arch alignment and stability with consistent improvements in key
radiographic parameters.

The surgical overview covers the following: patient positioning, evaluation of spring ligament insufficiency and procedure selection using tendoscopy, assessment and management timing of gastrocnemius–Achilles tendon contracture, preparation of the calcaneocuboid joint, and medial exposure for flexor digitorum longus tendon (FDLT) transfer. Two novel techniques developed by the author are introduced: corrective repair of the spring ligament (CORRS), a repair method addressing forefoot varus associated with spring ligament damage; and spring ligament reconstruction by remnant posterior tibial tendon (PTT) preservation (SREPP), a reconstruction technique that uses remnant PTT tissue to treat extensive spring ligament ruptures and concurrently correct forefoot varus. The article also offers surgical tips for FDLT transfer, bone grafting, and lateral column lengthening (LCL) via calcaneocuboid distraction arthrodesis (CCDA), with special attention to avoid residual forefoot varus.

Correction of midfoot and forefoot varus is emphasized as a crucial element in performing LCL for stage 1 PCFD. CORRS and SREPP are presented as effective adjunctive techniques that enhance the outcomes of CCDA by preventing over-lengthening and residual forefoot varus.

Spring ligament repair with and without augmentation demonstrates favorable outcomes in progressive collapsing flatfoot disorder: A systematic review

Background: Spring ligament disruption is a primary contributor to the development of progressive collapsing flatfoot deformity (PCFD). The purpose of this systematic review is to evaluate the radiographic findings, clinical outcomes, complications, and failures following spring ligament repair with and without augmentation.

Methods: During December 2025, the PubMed, Cochrane, and EMBASE library databases were systematically searched to identify studies examining radiographic findings, clinical outcomes, complications, and failures in patients who underwent spring ligament repair with and without augmentation.

Results: Nine studies including 209 patients (212 feet) with spring ligament injuries were analyzed. Patients who underwent spring ligament repair with and without augmentation were evaluated. Both treatment approaches were associated with clinically meaningful improvements in radiographic alignment, with mean talo-first metatarsal angle (TFMA) correction of 12.9° following isolated repair and 11.0° following repair with augmentation. Clinical outcomes also improved, with mean increases in American Orthopaedic Foot and Ankle Society (AOFAS) scores of 24.1 and 29.8 points for isolated repair and repair with augmentation, respectively. Complication rates were low for both isolated repair (4.5 %) and repair with augmentation (6.7 %), with corresponding failure rates of 1.3 % and 3.3 %, respectively.

Conclusion: Spring ligament repair with and without augmentation is associated with meaningful improvements in radiographic alignment and clinical outcomes in patients with PCFD. Across the included studies, both treatment approaches also demonstrated low complication and failure rates. While suture augmentation may provide additional mechanical support in select patients, the available evidence precludes direct comparative conclusions regarding the superiority of the technique over isolated spring ligament repair. Further high-quality, comparative studies are warranted to definitively establish the optimal surgical method for managing spring ligament injuries.
