Functional units in the human foot provide a meaningful basis for subdivisions of the entire foot during gait analysis as well as justified simplifications of foot models. The present study aimed to identify such functional units during walking and slow running. An invasive method based upon reflective marker arrays mounted on intracortical pins was used to register motion of seven foot bones. Six healthy subjects were assessed during walking and four of them during slow running. Angle-angle diagrams of corresponding planar bone rotations were plotted against each other and used to establish functional units. Individual functional units were accepted when the joints rotated temporally in phase and either (i) in the same direction, (ii) in the opposite direction, or (iii) when one of the two joints showed no rotation. A functional unit was generalized if all available angle-angle diagrams showed a consistent pattern. A medial array from the navicular to the first metatarsal was found to perform as a functional unit with parts rotating in the same direction and larger rotations occurring proximally. A rigid functional unit comprised the navicular and cuboid. No other functional units were identified. It was concluded that the talus, navicular, and medial cuneiform should neither be regarded as one rigid unit nor as one segment during gait analysis. The first and fifth metatarsals should also be considered separately. It was further concluded that a marker setup for gait analysis should consist of the following four segments: calcaneus, navicular-cuboid, medial cuneiform-first metatarsal, fifth metatarsal.

Quantifying rearfoot-forefoot coordination in human walking.

A method is proposed to facilitate the quantification and interpretation of inter-joint/-segment coordination. This technique is illustrated using rearfoot-forefoot kinematic data. We expand existing vector coding techniques and introduce a set of operational terms through which the coordinative patterns between the rearfoot segment and the forefoot segment are summarized: in-phase, anti-phase, rearfoot phase and forefoot phase. The literature on foot mechanics has characterized the stable foot at pushoff by a decreasing medial longitudinal arch angle in the sagittal plane, which is accompanied by forefoot pronation and concurrent rearfoot supination-in other words, anti-phase motion. Nine skin markers were placed on the rearfoot and forefoot segments according to a multi-segment foot model. Three healthy subjects performed standing calibration and walking trials (1.35ms(-1)), while a three-dimensional motion capture system acquired their kinematics. Rearfoot-forefoot joint angles were derived and the arch angle was inferred from the sagittal plane. Coupling angles of rearfoot and forefoot segments were derived and categorized into one of the four coordination patterns. Arch kinematics were consistent with the literature; in stance, the arch angle reached peak dorsiflexion, and then decreased rapidly. However, anti-phase coordination was not the predominant pattern during mid- or late stance. These preliminary data suggest that the coordinative interactions between the rearfoot and the forefoot are more complicated than previously described. The foot is responsible for the bodyweight transfer to the ground, while adapting to different terrains and activities. Despite this fundamental role, the knowledge about the foot bone intrinsic kinematics is still limited. The aim of the study is to provide a quantitative and systematic description of the kinematics of all bones in the foot, considering the full range of dorsi/plantar flexion and pronation/supination of the foot, both in weightbearing and nonweightbearing conditions. Bone kinematics was accurately reconstructed for three specimens from a series of computed tomography scans taken in weightbearing configuration. The ground inclination was imposed through a set of wedges, varying the foot orientation both in the sagittal and coronal planes; the donor body-weight was applied or removed by a cable-rig. A total of 32 scans for each foot were acquired and segmented. Bone kinematics was expressed in terms of anatomical reference systems optimized for the foot kinematic description. Results agree with previous literature where available. However, our analysis reveals that bones such as calcaneus, navicular, intermediate cuneiform, fourth and fifth metatarsal move more during foot pronation than flexion. Weightbearing significantly increase the range of motion of almost all the bone. Cuneiform and metatarsal move more due to weightbearing than in response to ground inclination, showing their role in the load-acceptance phase. The data here reported represent a step toward a deeper understanding of the foot behavior, that may help in the definition of better treatment and medical devices, as well as new biomechanical model of the foot.
