Simulated weight reduction using an anti-gravity treadmill - A pilot study of the impact of weight loss on foot and ankle arthritis

Background: Despite a paucity of evidence, obesity is frequently cited as an exacerbator of symptoms in foot and ankle arthritis. The aims of the current study were to determine whether simulated weight loss would improve symptoms in obese patients with foot and ankle arthritis.

Methods: Patients walked on an "anti-gravity" treadmill allowing simulated weight reduction. Pain was recorded at baseline weight and then compared with pain at simulated normal BMI.

Results: Simulated reduction to BMI 25 caused a significant reduction in pain. Mean pain scores improved from baseline to BMI 25 by 32% (15.9 points, p=0.04). Paired analysis showed a significant improvement in pain scores (p=0.016) from BMI of 30 to 25.

Conclusion: Simulated weight loss from high to normal BMI improved arthritic symptoms. This could be used to power future studies to further investigate the effects of weight loss in foot and ankle patients. Level of evidence Level II - repeated measures cohort study.

Purpose: Obesity is a growing health problem that affects a high percentage of the population. In podiatry context, few studies have addressed obesity because most pedobarographic systems are unable to bear the weight of patients with obesity, making it difficult to examine and manage these patients. The objective of this study was analyzed the sociodemographic characteristics, quality of life, foot disorders, and pedobarographic parameters of patients with extreme obesity who are candidates for bariatric surgery and determine the changes after weight loss post-surgery.

Materials and methods: We conducted a foot examination, a pedobarographic study using a Podoprint® pressure platform, and a quality of life questionnaire (EQ-5D) on 23 patients with extreme obesity and analyzed the changes 12-18 months after surgery in 11 of them.

Results: We observed foot disorders, high plantar pressure, greater rearfoot contact, flat footprint, asymmetries, and alterations in toe contact. Almost 73.9% of participants said they had foot pain, 56.5% said they had impaired mobility, and more than 40% said they had limitations in carrying out daily activities and suffered from anxiety. After weight loss, we observed improved quality of life; more foot disorders; changes in total contact area, plantar pressures, barycenter, contact time, and footprint; decreased pain perception, walking problems and anxiety situations. Moreover, medication decreased, but they need to take more vitamins and calcium.

Conclusion: Weight loss improved the quality of life of the participating patients but altered their foot disorders. All parameters need regular reassessment to detect changes and modify initially prescribed treatments.

Background
Bariatric surgery leads to considerable weight loss and improved glycaemic control and seems to have a favourable impact on diabetes related foot complications (DFC).

Objectives
To assess the effect of bariatric surgery on diabetes related foot complications in patients with type 2 diabetes and determine whether DFC symptoms are improved after bariatric surgery.

Methods
We searched MEDLINE, Embase and Cochrane Central Register of Controlled Trials. The primary outcome was the presence of DFC after bariatric surgery. The secondary outcome was the improvement of DFC after bariatric surgery among patients who already had DFC before surgery.

Results
There were nine studies showing the presence of DFC post bariatric surgery and six detailing the changes in DFC post bariatric surgery. Bariatric surgery was not associated with a lower risk of developing or worsening DFC compared to conventional medical treatment based on 4 randomised control trials (IR 0.87, 95 % CI, 0.26, 2.98), while from observational studies was associated with 51 % lower risk of DFC (IR 0.49, 95 % CI, 0.31, 0.77). Bariatric surgery was associated with improvement in diabetic neuropathy assessment parameters including toe tuning fork score, self-reported neuropathy symptoms, neuropathy symptom score, and neuropathy symptom profile.

Conclusion
Bariatric surgery led to a greater reduction in developing or worsening DFC among patients with type 2 diabetes compared to medical treatment in observational studies, but not among RCTs. Bariatric surgery was associated with improvements in diabetic neuropathy related assessment parameters and symptoms. Bariatric surgery could be a promising treatment for patients with type 2 diabetes who are at high risk of DFC.

AimTo compare the modifications in foot structure and shape in obese patients with or without type 2 diabetes mellitus (T2DM) after weight loss.MethodsWe studied all patients submitted to bariatric surgery in our Department between January and March 2019 with body mass index (BMI) between 40 and 48 kg/m2 before and after the weight loss, dividing them into two groups according to presence (Group 1) or absence (Group 2) of T2DM. We compared changes in: skin temperature at first and fifth metatarsal joint (MJ), ultrasound skin, subcutaneous and fascia thickness, dorsal and ankle circumferences.ResultsWe enrolled 46 patients: 19 in Group 1 (M/F 11/8; mean age 48.2 ± 10.3 yrs) and 27 in Group 2 (M/F 10/17, mean age 46.8 ± 10.0). Time requested for expected weight loss was longer in Group 1 (461.5 ± 131.3 days vs 334.2 ± 106.4, P = .0011). Foot and leg dimensions were decreased in both groups, but reduction was greater in Group 2. Also the reduction of skin temperature was greater in Group 2 both at first and at fifth MJ (P = .03 and .02). On the contrary ultrasound skin and subcutaneous thickness reduction was significantly greater in Group 1 both dorsally (P = .009) and plantarly (P < .05) as well as fascia thickness (P = .02).ConclusionsOur data confirm structural body changes induced by weight loss in severely obese patients, with different patterns in patients affected or not from T2DM: a more important decrease in tissue thickness in the former and a greater volume reduction in the latter, probably related to different oedema resorption

Differential Radiographic Response of Sagittal Foot Alignment to Early Weight Loss Following Sleeve Gastrectomy

Background and Objectives: We aimed to evaluate early postoperative radiographic changes in sagittal foot alignment following laparoscopic sleeve gastrectomy and to investigate the association between early weight loss and sagittal foot alignment parameters. Materials and Methods: This study included 72 consecutive patients who underwent primary laparoscopic sleeve gastrectomy. Standardized lateral foot radiographs were obtained preoperatively and at the fourth postoperative month. Meary's angle, calcaneal pitch, and talar declination angle were measured on all radiographs. Demographic and clinical variables, including age, sex, height, body weight, and body mass index (BMI), were recorded. Results: Meary's angle demonstrated a significant postoperative decrease from 15° (IQR, 8°) to 11° (IQR, 12°) (p < 0.001), indicating improvement in medial longitudinal arch alignment. In contrast, no significant postoperative changes were observed in the calcaneal pitch (p = 0.227) or talar declination angles (p = 0.751). The proportion of patients within the normal range for all measured sagittal alignment parameters increased postoperatively, without showing statistical significance. Statistical analysis revealed that all postoperative sagittal alignment parameters showed significant correlation with preoperative values. Notably, postoperative Meary's angle demonstrated a very strong positive correlation with preoperative Meary's angle (r = 0.80, p < 0.001), whereas no significant correlation was identified between postoperative Meary's angle and either postoperative weight or weight/BMI loss (p > 0.05). Although BMI loss showed a significant correlation with postoperative calcaneal pitch and talar declination angles, these correlations were weak to moderate (r = -0.403, and r = -0.362, respectively). Conclusions: Early postoperative body weight/BMI loss following sleeve gastrectomy is associated with modest, parameter-specific improvements in sagittal foot alignment, primarily reflected by changes in Meary's angle, suggesting that the medial longitudinal arch may be more responsive to early postoperative unloading than other sagittal alignment parameters. The strong association between preoperative and postoperative measurements underscores the central role of baseline alignment in determining early postoperative outcomes.
