    Hi

    I have an assignment on

Basing your account on the different aetiologies of hallux valgus, discuss the main lines of management of this deformity.

I have been searching journals for non surgical management and it is pretty hard finding the surgical was the easy bit. If any one can recommend any articles for conservative management it would be appreciated

Also if you don’t mind what are your ideas on the conservative managements

    Thank you

Run a search through this forum on hallux valgus and bunions and there should be enough reading, opinions and references to keep you going for days.

We have just implemented a thread tags system that is far from complete, but you might want to start by checking the bunions and hallux valgus tags.

In the unlikely event you can locate scientific papers on the related subject you may have to do a manual search through the podiatry journals that are not on Index Medicus.

Much depends on your agreed definition of surgery (I would check with your tutor) , ie is it open, closed or both.? Closed surgical technique would include foot orthoses with Kilmartin's work the most obvious refereed source. If closed surgical techniques are out of bounds for your assignment then you may need to think laterally to generate key terms such as 'traction' and the workings of the the 'pull and set brigade' and night 'serial" splinting e.g. Budin et al. This would certainly take you to Wolf and Davis Laws and how these may be applied to corrective treatments, such as manipulation etc. Try physio/physical therapy literature as a starting place.

Are non surgical approachs to HAV effective? Conditions must apply and much depends on the individual cases as well as the definiton and severity of Hallux Abducto-Valgus. Speaking personally I would doubt their efficiency. (although I would qualify this by saying so patients feel they are doing something and that makes them feel better )

Does surgical treatment have a benefit? Yes, but a significant large percentage of the HAV will return.

But hey what do I know?

thank you that was a great help

regards to the surgery part of the management he dose mean more open side of it, i believe it because i am on a undergraduate course, and will not being doing surgery when i first quailify, also the surgical side of things is covered in more detail in the final year of my degree

i will strart going through the lit from the physio side of things. say that this site has had a few good finds for me here just finding the orginal source is the hard part, the joys of being a
    student hey.

the comment of pt feeling that they are doing something i know what you mean, some of pt at the uni clinc believe they have not had a proper tx untill they have got some semi compressed felt.

the other annoying thing is that i only have 1500 words to write this essay.

    cheers

That's the easiest thing, just make clever use of labelled tables/diagrams and the Appendix. The words do not count !

    Good hunting

    Nick,

"Basing your account on the different aetiologies of hallux valgus, discuss the main lines of management of this deformity."

Have a look at the Cochrane Review - Ferrari J, Higgins JPT, Prior TD. Interventions for treating hallux valgus (abductovalgus) and bunions. Cochrane Database of Systematic Reviews 2004, Issue 1. Art. No.: CD000964. DOI: 10.1002/14651858.CD000964.pub2

    http://www.cochrane.org/reviews/en/ab000964.html

or get the full text via your university at:

It's a good start. Good luck finding aetiologies!
    Merry Christmas,
