Channel 4 Health freaks TV Show 21/10/2013

    Dear All,

    It was with some amusement that I decided to watch last nights program on Home Remedies.

    Of interest was the use of DUCT TAPE for the t/t of Verruccae.

    The 3 GP presenters were somewhat incredulous about this. But I thought to myself that when I trained at Plymouth 1983-1986 this was a common treatment regime along with all the other mysterious t/t's for our wonderful VP's.

    From memory we used: Sal Acid, Mono and Tri Chlor Acetic Acid, Pyrogallic Acid, Podophyllum Gel, Nitrous oxide gas. Liquid Nitrogen and 95% AgNO3. Plus a few others I have forgotten.

    Also we used occlusive tape such as DUCT Tape.

    When I graduated I ran the NHS Wart Clinic for 5 years, joy I know, and used many t/t initially.

    But after a short time I relied upon 95% AgNO3, Nitous Oxide Gas, Liquid Nitrogen and one that anecdotally worked really well on young KIDS Auto Suggestion. I asked them to every night and morning to tell the VP to go away.

    Now anecdotally all the t/t's were equally effective/ineffective.:butcher:

    But I would say that Liquid Nitrogen would always be my T/T of choice. But in young kids 95% AgNO3 or the auto suggestion.

    I was not a real fan of occlusal tape such as Duct Tape my self.

    Now the program presenters thought DUCT Tape was nuts and said no evidence. Also they had not heard of it so they said.

    This t/t went to the program's clinical trial and a measurable benefit was found.:drinks

    Sadly they did not mention Podiatry or Podiatrists AT ALL. Also they seemed to have little knowledge of VP/warts on the foot.

    So here I am in my 27th year Post Grad and nothing has changed in the way that the general public media deal with our small profession.:bang:

    perhaps the presenters should be directed to this site and do a simple search.

  3. How many studies have there now been on duct tape in the treatment of warts and what were the conclusions of each of these?

    Craig,

    Duct tape was the medium used to create an occlusive dressing. I have over the years never been convinced that occlusal is a good therapy. But as I am sure you are aware that many have used it and claimed a therapeutic benefit. They have even claimed that the VP of many years standing has resolved due to using an occlusive dressing.

    Still I believe that the best t/t for warts and VP is NOTHING. 90% of warts VP will resolve spontaneously over a 12-24 month period.

    Sadly many want a tt that will fix it NOW. I do not know of any tt modality that will resolve a wart VP.

    The only one that I know may have an impact is the administration of Gardasil to young girls and it has been reported that some of these girls had warts VP and these resolved after the administration of Gardasil.

    So warts and VP are no really further forward than when I trained. Apart from the fact that most tt's have gone in the bin.

    When I ran the NHS wart clinic I felt I was treating an aggrieved parent and not the child. Ho Hum. Even had a serious formal complaint from a parent that I would not tt her very young child. I was instructed to tt the kid. So I insisted that the head of dept was present. I then got the parent to sign a form stating that the tt was not necessary and that the tt was given at the expressed demand of the parent.

    The tt did involve discomfort for the poor kid as the parents knew this would be.

    I was very angry after this and said so.

    Also the parents then complained that the kid squealed .

    Fortunately this was 20 years ago and times have improved since in this matter.

    But programs like the one I described still annoy me as they are not well researched and do not include the Allied Health Professions.

    Mind you on the radio today a hypnotherapist was interviewed with respect to kids being traumatised by Clowns. This was because a Council asked a circus to remove 3 posters as they had had a SINGULAR complaint from somebody who had a phobia for clowns.

    Ha ha, I remember being slated last year for mentioning that I sometimes recommended duct tape as a treatment for plantar warts ( thanks guys!!). I was a new graduate and had been taught this as a credible treatment option at university. Duct tape is even listed as a treatment on the NHS website http://www.nhs.uk/Conditions/Warts/Pages/Treatment.aspx. I watched last nights programme and was shocked that they 'trialled' it, because, prompted by my grilling (!!) I have read the research and some studies do show it to be beneficial. However the bad boy of trials, A DOUBLE-BLIND RTC shows it is no more beneficial than moleskin.

    I cannot believe there was no mention of podiatry. The programme made me so angry!!!

    One of the "researchers" for this programme contacted me a couple of months ago requesting a list of "home remedies" for VP. I guess they decided not to air my response. When I'm back in the UK I'll post a copy of our email dialogue.

    A few. However, THE duct tape`Focht study`, which is much (mis)quoted, did not examine their patients during follow-up. Latest Cochrane reports on 2 further trials;

    Unfortunately, I missed Ray`s lecture as I erroneously made the decision to attend the `Latest Laser tx in Podiatry` which was on at the same time (although I did get a chance to chat with Ray over coffee. Nice chap). Whilst the Laser talk may have had some pertinent info, what irked me was the frequent referral to VP`s "having their own blood supply" which is, quite frankly, Chuffin Bobbins.

    ....and don`t get me started on Dr Pixie. She who is the face of a Pod supplier and recommends that pts should "moisturise between their toes..":bang:

    In all fairness Bel, and speaking as a totally un-biased male Podiatrist, how could someone called "Dr Pixie" possibly be wrong?

    On a separate note - it worried me, for a little while, how so many members of one particular high-profile Premiership soccer club would be fitted for so many orthoses which could not possibly fit into soccer boots.
    "Is she good-looking?" I asked the rep. "She's quite smart......." came the reply
    Then i Googled her! I'm neither saying nor infering anything whatsoever.

    but Dr Pixie - yay!:D

    Hi David,Mark from Ireland .Iwas watching also Ch 4 show.Even better Ive heard the use of Banana skins.Sadly no mention of ref to Chiropody/Podiatry Clinics,Always to the Lovely clebs GPS.Wake up you clowns . Many thanks Mark

    I took up yoga and over a 12-24 month period all my VPs were healed by it!!!

    On the tape though if big dollops of 60% sal acid aren't enough to guarantee resolving an entrenched VP through maceration of the tissue there is no way that a bit of maceration through occlusion by tape will work. Unless the patient uses it until the VP resolves itself of course.

    AKA spontaneous resolution.

    I agree with your thoughts on occlusion. The latest Cochrane review stated; "Duct tape demonstrated no advantage over placebo"

    You`ve stated this very specific figure before, yet I can`t find any evidence to suggest this. Can you tell me where you heard/read this and, more importantly, to which population this refers?

    When I was training we had a similar experience when the program embarrising bodies where they sent a patient for nail surgery by an orthopaedic surgeon. They chose this option over the well established private pods, the NHS and my own University all of whom were available. My lecturer used it as a what not to do for nail surgery.

    Patient poorly suitable due to obvious self neglect issues (the reason why she had the problem in the first place really).
    Surgery done in a manner which my lecturer said he'd fail us if we had done that.
    Improper follow up (both generally and because of the obvious self neglect issues the patient had).

    I think my lecturer made a formal complaint at the time
