Establishing the Minimal Clinical Important Difference and Minimal Detectable Change for the Cumberland Ankle Instability Tool

Objective
The objective was to establish the minimal detectable change (MDC) and minimal clinically important different (MCID) for the Cumberland Ankle Instability Tool (CAIT) in a population with chronic ankle instability (CAI).

Design
Experimental cohort.

Setting
Laboratory.

Participants
A convenience sample of fifty participants with CAI participated (12 males, 38 females, monthly giving-way=5.84±12.54). CAI inclusion criteria included a history of an ankle sprain, recurrent episodes of giving way, and a CAIT score ≤ 25.

Interventions
Participants completed an injury history and demographic data then the CAIT. Participants then either participated in 4 weeks of wobble board balance training, resistance tubing strength training, or no intervention. After 4 weeks, participants re-completed the CAIT and recorded their global rating of change (GRC).

Main Outcome Measures
Dependent variables were pre- and post-intervention score on the CAIT, and post-intervention GRC. MDC with 95% confidence was calculated. A receiver operating characteristic curve (ROC) identified the optimal CAIT cut-point (MCID) between improved and unimproved individuals based on their GRC. Area under the curve (AUC) was used to identify a significant ROC curve (alpha = 0.05).

Results
The average CAIT score pre-intervention was 16.8±5.6, and post-intervention was 20.0±5.2. Thirty-one participants (62%) rated themselves as improved on the GRC, whereas 19 (38%) were not improved. The ROC was significant (AUC = 0.797, p=0.001), indicating that CAIT change score significantly predicted clinical status. The MDC was 3.08, the MCID was ≥3 points.

Conclusions
The CAIT has a MDC and MCID of ≥3 points. When CAIT scores are utilized to assess patient change over time, these scores should be used as a minimum threshold to indicate detectable and clinically meaningful improvement.

Objective
The purpose of this study was to investigate the association between Cumberland Ankle Instability Tool (CAIT) scores and postural stability during a diagonal landing, and to investigate whether postural stability is altered in collegiate soccer players with and without functional ankle instability (FAI). Design: Cross-sectional study.

Setting
Laboratory.

Participants
Ninety-one soccer players were classified into a FAI group (history of at least two ankle sprains and a CAIT-Japanese version score ≤25, n = 28), a copers group (history of one ankle sprain and a CAIT-Japanese version score ≥26, n = 32), or a control group (no history of ankle sprain, n = 31).

Main outcome measures
Time to anteroposterior stabilisation (TTSAP) and mediolateral stabilisation (TTSML) were measured during the diagonal single-leg landing.

Results
The CAIT scores were correlated with TTSAP (P < 0.05, rs = −0.214) and TTSML (P < 0.01, rs = −0.566). TTSAP was longer in the FAI group than in the control group, and TTSML was longer in the FAI group than in the other groups.

Conclusions
Our findings indicate the presence of an association between the CAIT-J score and TTSML, as well as postural stability deficits in collegiate soccer players with FAI during diagonal landings.

Background
Patient reported outcome measures (PROMs) are essential for evaluating treatment of ankle instability (AI). The aim was to assess the content validity and the measurement properties of all relevant PROMs for AI.

Methods
Relevant PROMs were identified from PubMed and SCOPUS. The development and validation quality of the PROMs was assessed according to established scientific standards.

Results
Seventeen PROMs and 56 validation studies were analyzed. Content validity, which ensures the PROM measures what is relevant, is obtained by involving target patients in the development process. Only three PROMs identified had some degree of patient involvement (Cumberland Ankle Instability Tool (CAIT), Lower Extremity Function Scale (LEFS), and the Foot and Ankle Ability Measure (FAAM)). Of these, only FAAM was somewhat rigorously validated using modern psychometric validation methods, and exhibited superior measurement properties (construct validity).

Conclusion
No existing PROM is completely adequate to evaluate AI. However, FAAM is the best choice.

Systematic review.

Background and objective
The International Ankle Consortium developed a core outcome set for the assessment of impairments in patients with lateral ankle sprain (LAS) without consideration of measurement properties (MP). Therefore, the aim of this study is to investigate MPs of assessments for the evaluation of individuals with a history of LAS.

Methods
This systematic review of measurement properties follows PRISMA and COSMIN guidelines. Databases Pubmed, CINAHL, Embase, Web of Science, Cochrane Library and SPORTDiscus were searched for eligible studies (last search: July 2022). Studies on MP of specific tests and patient-reported outcome measurements (PROMs) in patients with acute and history of LAS (>4 weeks post injury) were deemed eligible.

Results
Ten studies of acute LAS and 39 studies of history of LAS patients with a total of 3313 participants met the inclusion criteria. Anterior Drawer Test (ADT) in supine position five days post injury and Reverse Anterolateral Drawer Test are recommended in acute settings in single studies. In the history of LAS patients, Cumberland Ankle Instability Tool (CAIT) (4 studies) as a PROM, Multiple Hop (3 studies) and Star Excursion Balance Tests (SEBT) (3 studies) for dynamic postural balance testing showed good MPs. No studies investigated pain, physical activity level and gait. Only single studies reported on swelling, range of motion, strength, arthrokinematics, and static postural balance. Limited data existed on responsiveness of the tests in both subgroups.

Conclusion
There was good evidence to support the use of CAIT as PROM, Multiple Hop, and SEBT for dynamic postural balance testing. Insufficient evidence exists in relation to test responsiveness, especially in the acute situation. Future research should assess MPs of assessments of other impairments associated with LAS

Study design: Evaluation of the psychometric properties of a translated, culturally adapted questionnaire.

Objective: Translating, culturally adapting, and validating the Italian version of the Cumberland Ankle Instability Tool (CAIT-I).

Summary of background data: Ankle sprains are one of the most common musculoskeletal injuries and can lead to chronic ankle instability (CAI). The International Ankle Consortium recommends the Cumberland Ankle Instability Tool (CAIT) as a valid and reliable self-report questionnaire assessing the presence and severity of CAI. At this moment, there is no validated Italian version of CAIT.

Methods: The Italian version of the CAIT (CAIT-I) was developed by an expert committee. Test-retest reliability of the CAIT-I was measured in 286 healthy and injured participants within a 4-9-day period, by using Intraclass Correlation Coefficients (ICC2,1). Construct validity, exploratory factor analysis, internal consistency and sensitivity were examined in a sample of 548 adults. Instrument responsiveness over 4 time points was determined in a subgroup of 37 participants.

Results: The CAIT-I demonstrated excellent test-retest reliability (ICC≥0.92) and good internal consistency (α = .84). Construct validity was confirmed. Identified cut-off for the presence of CAI was 24.75, with sensitivity= 0.77 and specificity= 0.65. There were significant differences across time for CAIT-I scores (P < .001), demonstrating responsiveness to change, but no floor or ceiling effects.

Conclusion: The CAIT-I demonstrates acceptable psychometric performance as a screening and outcome measure. The CAIT-I is a useful tool to assess the presence and severity of CAI.

Cross-cultural adaptation, reliability, and validity of the Turkish version of the Cumberland Ankle Instability Tool

Objectives: The objective of the study was to cross-culturally adapt and assess the validity and reliability of the Turkish version of the CAIT (CAIT-TR).

Methods: The CAIT was translated and adapted into Turkish according to accepted cross-cultural adaptation guidelines of self-reported measures. A total of 130 individuals, including healthy participants (n = 40) and with chronic ankle instability (CAI) (n = 90), were recruited in this study. The internal consistency and test-retest reliability of the CAIT-TR were assessed using Cronbach's alpha and intraclass correlation coefficients (ICC), respectively. To assess convergent validity, hypotheses were tested regarding expected correlations between CAIT-TR, Foot and Ankle Outcome Score (FAOS) subscales, and the Visual Analogue Scale (VAS). Discriminative validity was evaluated with the hypothesis that the CAIT-TR can distinguish between subjects with and without CAI and also calculated a cut-off score for CAI. The dimensional structure of the CAIT-TR was investigated with confirmatory factor analysis. Additionally, minimal detectable change (MDC), floor/ceiling effects, and measurement error values were determined.

Results: The CAIT-TR demonstrated high Internal consistency (Cronbach's alpha = 0.854) and test-retest reliability (ICC = 0.919). Regarding convergent validity, the CAIT-TR exhibited a moderate-strong correlation with both the FAOS and VAS. The confirmatory factor analysis supported the unidimensional structure of the questionnaire. The identified cut-off value for the CAIT-TR was 25, and the MDC for individual-level CAIT-TR scores was determined to be 1.87. No floor or ceiling effects were observed.

Conclusion: The CAIT-TR is a valid and reliable questionnaire for the assessment of ankle instability within the Turkish population.
