Plantar ulcers in leprosy and self care

    I worked for the London Foot hospital from 1995 until 2003 as a bespoke shoemaker and made footwear for 13 cases for the Hospital for Tropical Diseases. During that time I researched leprosy quite a bit. It's still around, even in the UK. Multiple Drug Therapy kills it within days but the disability and disfigurment is perminent. Treat it like extreme diabetes with peripheral neuropathy. Full contact inserts plus protective footwear is essential.
    Two problems persist. 1) It is getting so rare that even in India, doctors may have never seen it and fail to diagnose it till it is advanced. 2) People wearing protective footwear in hot countries are often quickly suspected by others as having leprosy. Hence they wear ordinary sandals to hide their condition, hence another reason for poor care for their ulcers.

    In Toronto we had a number of Hansen's clients. It was seen by the client as a "stigma" within their community. Often having the leg amputated was a preferred outcome for them as they could explain this away as an "accident", therefore not having to deal with the cultural stigma of the disease.

    very interesting information regarding Hansen's, but I need to ask, I can find info on the complications and the foot problems caused by HD but am struggling to find what a pod can do for a patient. All I have found is supportive custom made footwear, splints etc in cases of foot drop. what more is there that can be done to assist those with associated feet problems? Currently researching for an assignment.

    am working with a Leprosy program in Thailand at the moment and there is still so many cases even in a 9 year old girl. The stigma like Bill mentioned is disheartening to watch as people are evicted from their villages by even close family.

    Any one have any tips regarding care other than diabetes like care for neuropathy and footwear?

    There was a television programme here in the UK last week about this disease in India. The multiple drug therapy kills the bacilli within a very few days. The main problems they outlined were as you have found, the disfigurement the disability and the social exclusion. The emphasis carried out by the team shown in the programme was on education within the community from which the people affected by leprosy had been excluded. This involved actually going into the communities with translators and taking the excluded people with them. Not an easy solution I must say but if it can be done, it would be better than anything else.
