Extracorporeal shock wave therapy with custom foot orthotics in plantar fasciitis treatment

    - red flag re randomization
    - within groups rather than between groups analysis
    - multiple comparisons (data fishing)
    - no intention to treat analysis (orthotics could have been better if they did that)
    - nothing on the appropriateness of dose of shock wave therapy
    - nothing on foot orthotic dosing issue (custom foot orthotic appear to be total contact type devices with none of the specific design feature that I would use for plantar fasciitis)
    - both groups were given stretching - both groups could have got better for that reason
    - no 'no treatment group' or placebo group, so both groups could have improved as part of the natural history

    The study is a joke! As an early adopter of HIGH energy ESWT (Dornier EPOS Ultra), my experience, and the experiences of those in the group of DPMs using it, in my geographical region, was near 90% improvement/cure at 6 months. Orthotics are as much a treatment, as they are for preventive measures. It is definitely not one or the other. Prescription functional foot orthotics are the first modality to use. At 4 -6 mos, if the pain has become recalcitrant, ESWT should be used.

    Plantar fasciitis is not a degenerative condition of the plantar fascia; plantar fasciosis is. Orthotics potentially treat underlying causes of plantar fasciitis while ESWT is used to treat plantar fasciosis. Flawed premise for making a comparison.

    No, really? :) When using the Dornier EPOS Ultra, it has an ultrasound imaging head. It allows the user to see the area of abnormal thickening (over 3 mm), and makes it easy to treat that area. On occasion, the abnormal thickening can extend up to 6-7 cm distal. The Dornier EPOS Ultra needs to treat the entire abnormal area. Low energy devices don't really cure.
