THE IMPACT OF ACHILLES TENDON RUPTURE ON THE STRUCTURE AND FUNCTION OF THE ACHILLES TENDON AND PLANTARFLEXORS AFTER NON-SURGICAL MANAGEMENT: A CROSS-SECTIONAL STUDY

Introduction

Acute Achilles Tendon Ruptures (ATR) cause lasting muscular deficits and impair function and quality of life. This study aimed to understand recovery post rupture by examining tendon structure using ultrasound tissue characterisation (UTC), isometric plantarflexor strength, physical activity and patient reported outcomes (PROM).
Methods

Cross-sectional study design consisting of 90 participants. Data were collected from 15 participants at six different rehabilitation timepoints (0, 8, 10 weeks, 4, 6, 12 months). Participants were recruited from a National Health Service clinic using non-surgical management.
Findings

Participants mean (SD) age 48 years (16), 91% male, body mass index 29kg/m2, 54% white British with a median of 1 comorbidity. Primary mechanism of injury was sport (71.1%). Deep vein thrombosis rate was 9.3%.

Based on UTC, ruptured tendon cross-sectional area (CSA) was 287.55 mm2 at 10 weeks, 203.62mm2 at 12 months. Disorganised fibrillar structure was 32% lower at 12 months than 10 weeks. Disorganised fibre percentage was consistent at each assessment point (10 weeks:32%, 12 months:30%).

Isometric plantarflexor strength on the ruptured limb at 12 months was 61.3kg (20.8) or 0.7x body weight (BW) whilst the non-ruptured limb was 93.3kg (29.5) or 1.1x BW. Daily steps increased from 3720 (1889.8) at week 0 to 9048.4 (2750.1) at 12 months.

Conclusion

There is substantial remodelling of the tendon during the initial 12 months post ATR, with tendon CSA differing 29% across assessment points. Proportion of disorganised collagen remains consistent from 10 weeks to 12 months post ATR, whilst CSA reduces. Individuals presenting with ATR managed non-surgically have a 34% or 0.4xBW isometric strength deficit at 12 months and still present with fear of movement and reduced function based on PROMs.

Introduction and aim of study: Achilles tendon rupture (ATR) is a significant injury, particularly for athletes. If not managed properly, it can lead to complications and hinder athletic performance. This study aims to synthesize existing knowledge on risk factors, treatment, and recovery associated with ATR, helping identify the optimal treatment pathway for a prompt return to sports. It will also highlight prevalent risk factors, aiding in injury prevention for patients and healthcare professionals.

Materials and methods: This literature review summarizes data from PubMed, Google Scholar, and other scientific sources on ATR. Key search terms included “Achilles tendon rupture treatment,” “risk factors,” “Achilles tendon injuries,” and “physiotherapy in ATR.”

Results: Various risk factors for ATR were identified, with some later proven irrelevant, while others remained significant. Treatment options include both non-operative and operative management, with minimally invasive techniques showing the best outcomes, however each method can be employed in specific indications. The recovery period typically lasts around six months, with rehabilitation being a key component. Other alternative methods and their benefits are being explored.

Conclusion: Optimal outcomes for ATR treatment are achieved through operative intervention, particularly minimally invasive surgery, paired with early rehabilitation. Physical therapy, blood-flow restriction training, and stretching can aid return to sport. Considering risk factors in all patients, especially athletes in contact sports, is crucial. The correct management of Achilles tendon rupture, consisting of early diagnosis, surgical treatment and appropriate rehabilitation, allows recreational athletes to return to their previous sports with a level of performance similar to that prior to the injury.

Serum and Urinary Proteomic Signatures Revealing Redox and Metabolic Dysregulation in Acute Achilles Tendon Rupture

Objectives: The etiology of acute Achilles tendon rupture (ATR) remains elusive. A comprehensive case-control study of the proteome profile was conducted to gain insights into the potential pathogenesis of acute ATR and identify novel biomarkers.

Method: Serum and urine samples were collected from 15 acute ATR patients and 15 healthy individuals for proteome analysis. Protein levels were assessed using isobaric tags for relative and absolute quantitation (iTRAQ) for serum and label-free proteomic approaches for urine. Differential expression was considered significant at levels exceeding 2-fold (for urine) and 1.2-fold (for serum), with a p-value below 0.05.

Results: In serum and urine samples, 44 and 198 proteins were identified, respectively, which exhibit significant differences between acute ATRs and normal Achilles tendons. Our bioinformatics analysis revealed the involvement of multiple biological processes and pathways in acute ATRs, including immune response, metabolism, and redox regulatory pathways. Several differentially expressed proteins were found to participate in various metabolic pathways, suggesting their potential pivotal roles in ATR pathogenesis. Furthermore, protein-protein interaction analysis of the differentially expressed proteins (P<0.05) highlighted abnormalities in major protein-protein interaction hubs, specifically pyruvate kinase (PKM), peroxiredoxin-1 (PRDX1), phosphoglycerate kinase 1 (PKG1), profilin-1, and apolipoprotein A-IV, observed in the serum and urine samples of acute ATR patients.

Conclusion: Abnormal nitrogen, carbohydrate, glucose, and long-chain fatty acid metabolic proteins, as well as PKM, PRDX1, and PGK1 abnormalities, may be involved in the pathogenesis of acute ATR.

Background

Acute Achilles tendon rupture (AATR) surgical repair debates center on the clinical efficacy of minimally invasive surgery (MIS) versus open surgery (OS), with immobilization duration poorly stratified. This prospective cohort study aimed to compare clinical outcomes of OS and MIS for AATR repair and evaluate the impact of immobilization duration (0, 2, or 4 weeks) on postoperative rehabilitation.
Methods

A total of 474 patients undergoing surgical repair for acute AATR were stratified into six groups based on surgical approach (OS: 265 cases; MIS: 209 cases) and immobilization duration (0, 2, or 4 weeks). The primary outcomes were postoperative complications, while secondary outcomes included recovery times for Achilles tendon function. Data regarding the operative times, incision lengths, the visual analog scale (VAS) score, the Achilles tendon Total Rupture Score (ATRS), and the American Orthopaedic Foot and Ankle Society (AOFAS) Ankle-Hindfoot Scale score, and the relative Achilles tendon resting angle (ATRA) were also collected.
Results

MIS groups demonstrated significantly shorter operative times (34.1–34.4 vs. 45.1–46.1 min, P < 0.001) and reduced incision lengths (2.2–2.4 vs. 4.5–4.7 cm, P < 0.001) compared to OS. Postoperative VAS scores were markedly lower in MIS cohorts during the first 2 weeks (P < 0.001), with pain resolution comparable across all groups by 8 weeks. Despite the superior early functional recovery in PF (9.1–33.4 vs. 14.6–38.9 days, P < 0.001), Group D and E exhibited higher re-injury rates compared to OS (P < 0.05), in which Group D also demonstrated higher re-operation rates (5.6% vs. 0, P = 0.038). Prolonged immobilization (4 weeks) delayed functional recovery in both cohorts (P < 0.001). While transient differences in AOFAS Ankle-Hindfoot Scale and ATRS scores were observed at intermediate time points, all groups achieved near-maximal functional scores by 48 weeks, with no significant between-group differences (P > 0.05). Relative ATRA exhibited no significant intergroup differences at 48 weeks postoperatively (P > 0.05).
Conclusion

MIS for acute Achilles tendon rupture achieves faster early recovery but carries higher re-injury risks, mitigated by 4-week immobilization. OS benefits from shorter (2-week) immobilization. Both approaches yield equivalent long-term functions, emphasizing the need for tailored protocols and refined MIS techniques to optimize outcomes.

RETURN TO SPORT AFTER ACHILLES TENDON RUPTURE

Background: Achilles tendon (AT) rupture is a common injury, particularly among physically active individuals, with a 30% growth rate in the most affected age group (30–39 years). Among professional athletes, 25% fail to return to sport (RTS), and those who do typically require 8–11 months of recovery. Furthermore, their performance can decline by up to 50% during the two years following RTS, and their career length is shorter compared to non-injured players or those with anterior cruciate ligament (ACL) injuries. Long-term deficits after AT repair have been documented up to 10 years post-injury, including tendon elongation, reduced calf circumference, atrophy of type-1 muscle fibers, decreased heel-raise test performance, and reduced ability to generate force in a plantar-flexed position. Additionally, low psychological readiness to RTS, or high levels of kinesiophobia at six months may influence RTS outcomes. Given these suboptimal RTS outcomes and the lack of consensus on optimal treatment strategies for AT rupture, there is a need to explore potential areas for improvement in the RTS process. Aims: These considerations have inspired my PhD research project, which aims to identify gaps in the current AT RTS process and provide insights to optimize outcomes for patients recovering from AT rupture. The specific aims of the three research projects are as follows: 1. To map the literature regarding which criteria are used to clear athletes for RTS after AT repair. Understanding the criteria defined in the literature allows for clarity on the standards required for RTS and the identification of potential gaps area in the RTS process that lack clear guidelines. 2. To observe and analyze the impact of AT rupture in ankle movement quality during the mid-stage of rehabilitation. The comparison between the repaired and unrepaired limbs aims to identify potential differences in movement quality between the two ankles. 3. To describe the on-field rehabilitation (OFR) workload of football players returning to sport after AT repair. Global Positioning System (GPS) monitoring workloads during the late-stage of rehabilitation may be a key factor to lead load progression during the late-stage of rehabilitation, providing insights to prepare football players for the demands of the return to training (RTT). Methods: 1) Project one involved a Scoping Review conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) Checklist. A combination of search terms and Boolean operators, regarding AT rupture and RTS criteria, was used to search the literature from 1995 to 2022 across three databases: Web of Science, Scopus, and MEDLINE. Papers inclusion and exclusion criteria were defined. A data charting was create to collect criteria, and the insights regarding AT rehabilitation outcome. 2) Project two was a case series involving 23 sportive patients who underwent AT repair. The inclusion criteria for participants were: being treated at one of the Isokinetic Medical Group Clinics, having undergone Achilles tendon (AT) repair surgery, and performing a bilateral depth jump. To observe the ankle movement quality the ankle joint angle (γ angle), and peak vertical ground reaction force (peak VGFR), in both repaired and unrepaired limbs during the eccentric and concentric phase of a bilateral depth jump where assessed. To test for differences between the repaired and unrepaired sides, variables were analyzed collectively using a multivariate Mixed Linear Model with repeated measures. Effect sizes and confidence intervals were also computed. 3) Project three was a pilot study with a small cohort including 5 male football players who underwent AT repair and were monitored during the OFR. Workload data for each OFR session were collected using GPS and heart rate tracking. Variables measured and reported were: PL, Player load (Arbitrary Unit-AU); TD, total distance (km); MSR, moderate speed running distance (defined as 15-20 km/h); HSR, high speed running distance (defined as 20-25 km/h); SRT, sprint distance (>25 km/h); MS, max-speed (km/h); ACC, Number of accelerations (>2m/s2); DEC, Number of decelerations (>2m/s2); time in Aerobic zone (70-85% HRmax); time in Anaerobic zone (>85% HRmax). Variations in GPS metrics were calculated as percentages between stages and from stage 1 to stage 5 to identify notable changes. As a follow-up, the ankle ligament reconstruction-return to sport injury (ALR-RSI) score was used to evaluate the footballers psychological readiness to RTS. Main results: 1. In project one, the main result was that none of the 34 included studies explicitly defined any RTS criteria to clear athletes after AT rupture. The literature primarily focused on provide measurements related to the early stage of rehabilitation, while no specific measurements for the mid- and late-stages of rehabilitation were identified. These findings highlight the need to investigate appropriate measurements for evaluating progress during the mid-and late- stage of the rehabilitation process in the context of RTS after AT rupture. 2. In project two, the main findings were: an average ankle joint angle 8 degrees smaller for the limb that underwent AT repair compared to the unrepaired one at IC during the depth jump (p<0.001), and a mean peak VGRF 3.6 N/kg higher for the unrepaired limb at IC (p<0.001). These results suggest changes in ankle movement quality, with a more dorsiflexed repaired ankle and a body weight shift (BWS) towards the unrepaired limb during the landing phase of the jump, while no differences were observed during the concentric phase. Integrating ankle movement quality assessments in the mid-stage of rehabilitation may be a key factor in evaluating ankle impairments and potential shifts in ankle movement patterns with higher peak knee loads in the same limb. 3. In project three the main result was that a notable increase was found for RPE, MSR, HSR, ACC, DEC, aerobic and anaerobic zone from the start to the end of the OFR. These metrics may correlate with improved RTS outcomes and risk of re-injury. Thus, GPS load monitoring data plays a key role in the progress of the football player between OFR stages following AT repair. Additionally, suboptimal outcomes in psychological readiness for RTS may occur post- AT repair. Therefore, although RTS is achievable after AT repair, further research is required to optimize OFR load management and enhance psychological readiness for RTS.

Background

Prior studies have compared patient-reported outcomes between open repair and one of either the two minimally invasive techniques for Achilles tendon rupture: Percutaneous Achilles Repair System (PARS) and Midsubstance Speedbridge Implant System (MSB). However, no study has compared patient-reported outcomes measured by Patient Reported Outcomes Measurement Information System (PROMIS) physical function (PF) and PROMIS pain interference (PI) and the Achilles Tendon Total Rupture Score (ATRS) between PARS and MSB. Our study compared patient-reported outcomes measured by PROMIS and ATRS scores between PARS and MSB. We hypothesized that patient-reported outcomes would be similar between groups.
Methods

This was a retrospective review of 434 patients who underwent Achilles rupture repair from 2018 to 2023 at a single institution. Tendinopathies, open injuries, concomitant fractures, tendon transfers, gastrocnemius recessions, and open repairs were excluded. A total of 316 patients met inclusion criteria and were contacted to complete a postoperative questionnaire containing PROMIS and ATRS. 119 (78 PARS and 41 MSB) completed all surveys and were included for final analysis. Wilcoxon rank-sum and Kruskal-Wallis tests were used to assess differences in mean scores. Chi-squared and Fisher’s exact tests were used to compare incidence of complications. All tests were conducted at a significance level of α = 0.05.
Results

Average follow-up was 30 months at time of survey completion. There were no significant differences in PROMIS PF, PROMIS PI, and ATRS measures between groups (p > 0.05). Mean PARS PROMIS PF, PROMIS PI, and ATRS were 58.8, 44.2, and 86.0, respectively. Mean MSB PROMIS PF, PROMIS PI, and ATRS were 55.3, 44.0, and 82.5, respectively. No significant differences existed in incidence of each postoperative complication between groups (p > 0.05).
Conclusion

In the largest study to compare patient-reported outcomes between PARS and MSB, outcomes were similar between both groups. Both techniques resulted in PROMIS PF greater than the population mean and PROMIS PI lower than the population mean. Each had similar ATRS scores. Overall, both MSB and PARS were safe and effective strategies for surgically managing Achilles ruptures

Examining prior injury relative to Achilles tendon ruptures in National Basketball Association players

The purpose of this study is to examine Achilles Tendon (AT) rupture and prior injury in the National Basketball Association (NBA). Publicly available data was sourced via two online databases and filtered to identify those reported as having sustained AT rupture. A full injury history was then generated for those players. A total of 51 AT ruptures were identified. Thirty-five per cent (18) had previously missed at least one game for "Achilles tendinitis" or "Achilles tendon injury" on the side of the affected limb. A further 18% were noted to have suffered plantar fasciitis or foot inflammation (9) or previous ankle injury (9). Seven previous injuries were knee related, and five were described as calf muscle injuries. The remaining three had prior soft tissue injuries to the quadriceps, hamstring or groin. Understanding any potential association between previous small time-loss injuries and large time-loss injuries such Achilles tendon ruptures is an important step in advancing injury risk mitigation strategies in professional basketball.

Objective: The aim of this study was to compare the functional outcomes of patients who underwent Achilles tendon repair either
with biological technique or standard open technique.
Methods: A retrospective evaluation was made of 71 patients who were surgically treated for Achilles tendon rupture. The patients
were separated into 2 groups according to the surgical technique and type of rehabilitation. Group 1 comprised 20 patients who
underwent biological repair and early rehabilitation. Group 2 comprised 23 patients who were treated with a standard open surgical technique and late rehabilitation–early immobilization. The primary functional outcome measurements were the American
Orthopaedic Foot and Ankle Society (AOFAS) score and isokinetic strength measurements assessed by a Cybex NORM device.
Results: The mean time to return to daily activities was determined to be statistically significantly shorter in group 1 than in group 2
(P=.014). No significant difference was found between the groups in respect of the AOFAS scores of the operated side (P=.824).
A significant decrease was determined on the operated side compared to the normal side in the isokinetic strength measurements
of group 2 (P < .05).
Conclusion: According to the results acquired from this study biologic repair resulted in a significantly shorter time to return the daily
activities with similar clinical outcomes compared to the classic repair.

Serial ultrasound imaging leading up to an Achilles tendon rupture after identification of a thickened paratenon: a case report

A 22 year-old lacrosse student-athlete had participated in a research study examining the Achilles tendons of asymptomatic athletes, with normal appearance of his Achilles tendons bilaterally 7 months prior to injury. Around 4 weeks prior to injury, the patient started to report achy pain in the affected Achilles tendon, exacerbated by ankle dorsiflexion and plantarflexion, in the context of using new cleats during conditioning. He underwent repeat ultrasound assessment, identifying a normal appearing Achilles tendon, though with new hypoechogenic paratenon thickening approximately 1-4 cm proximal to the calcaneus. He ultimately suffered a non-contact complete Achilles tendon rupture. This case is the second known example of an Achilles tendon rupture with prior asymptomatic advanced imaging, and additionally calls into question the role of the Achilles paratenon in Achilles tendon pathology and rupture.

Correlation between functional and structural variables in patients undergoing achilles tendon surgical repair

Achilles tendon (AT) rupture promotes deficits in the plantar flexors and AT's structural properties, which have been related to deficits in the ankle joint range of motion (ROM), in the plantar flexors' ability to produce force and in functionality. The objective of this study was to evaluate possible correlations between the triceps surae's structural variables with performance in functional tests, and to compare these correlations between the uninjured limb and the limb that underwent AT surgical repair. Both legs from 28 participants (37 ± 5 years) were evaluated after tendon surgical repair. Structural variables included plantar flexors' cross-sectional area (CSA), muscle thickness (MT) and echo intensity (EI) and AT's CSA. Functional variables included ankle ROM, maximal isometric plantar flexion torque (at 10° of dorsiflexion) and the single leg hop test (SLHT, single leg triple jump test). Bivariate Pearson and Spearman correlations were performed, and the partial correlation test contained the time after surgical repair as a moderating variable. There was a correlation between medial gastrocnemius CSA and SLHT (r: -0.457 and p: 0.037) in the injured limb, and between medial gastrocnemius MT and torque in the uninjured limb (r: -0.521; p: 0.011). When we added time as a moderating variable, the same correlations were significant. The results of the correlations with and without the moderating variable do not support the above-mentioned causal hypothesis between AT rupture and functional deficits.

Influence of preoperative blood lipid profiles on postoperative recovery in patients with acute Achilles tendon rupture

Objective To investigate the influence of preoperative blood lipid levels on postoperative recovery in patients with acute Achilles tendon rupture (AATR). Methods The clinical data of 353 AATR patients treated in Department of Orthopaedics Trauma of our hospital from Jan. 2021 to Jan. 2024 were retrospectively analyzed, including general clinical information and preoperative fasting blood biochemistry indices. Patients’ Achilles tendon total rupture score (ATRS) and American Orthopaedic Foot and Ankle Society (AOFAS) ankle-hindfoot score were recorded 6 months postoperatively. Postoperative recovery was categorized as favorable if ATRS≥80 and poor if ATRS＜80; similarly, recovery was considered favorable if AOFAS score ≥90 and poor if AOFAS score ＜90. Univariate and multivariate logistic regression analyses were done to investigate the influencing factors of patients’ postoperative recovery. Results When categorized by ATRS, patients in the favorable postoperative recovery group exhibited significantly lower serum levels of triglyceride (TG), total cholesterol (TC), and low-density lipoprotein (LDL) compared to the poor postoperative recovery group (all P＜0.05). Similarly, when categorized by AOFAS score, the favorable postoperative recovery group had lower serum TG and TC levels than the poor postoperative recovery group (both P＜0.05). Univariate logistic regression analysis demonstrated that in the ATRS grouping, elevated serum levels of TG, TC and LDL were significantly associated with an increased risk of poor postoperative recovery in AATR patients (all P＜0.05). Similarly, in the AOFAS score grouping, elevated serum levels of TG and TC were significantly correlated with an increased risk of poor postoperative recovery in AATR patients (both P＜0.05). Multivariate logistic regression analysis further identified that high serum LDL level was an independent predictor of poor postoperative recovery in AATR patients (P＜0.05). Conclusion High preoperative serum LDL level is associated with an increased risk of poor postoperative recovery in AATR patients. Therefore, preoperative blood lipid management in AATR patients may have positive effects on postoperative functional recovery.

Outcomes Following Achilles Tendon Ruptures in the National Hockey League: A Retrospective Sports Database Study

Background: The purpose of this study was to evaluate Achilles tendon ruptures (ATR) in NHL players and the effects on return to play and player performance metrics. The incidence, mechanism of injury, management strategy, return to play (RTP), and post-injury were assessed from official online sports databases. Methods: A retrospective review of NHL players who sustained a partial or complete tear of the Achilles tendon from 2008 to 2024 was performed. Data were collected from NHL injury databases and media reports, and included player demographics, injury mechanism, treatment, and post-injury performance metrics. A Wilcoxon signed rank test was used to compare pre-injury and post-injury performance metrics, with significance set at p < 0.05. Results: Here, 15 NHL players with a mean age of 27.8 years were identified, with a prevalence rate of 0.125 injuries per 10,000 athletic exposures. Overall, 73.3% of ATRs were non-contact in nature, with 60.0% of ATRs occurring during off-season training. Fourteen players were managed with non-operative treatment, with no re-ruptures reported. The RTP rate was 93.3%, with players missing a mean number of 45.7 games. However, there was a deterioration in post-injury performance metrics, including games played per season, plus/minus rating, and time on ice per game post-injury. Conclusions: This study found that Achilles tendon ruptures are an uncommon injury in NHL players, with a prevalence rate of 0.125 injuries per 10,000 athletic exposures. A high RTP rate of 93.3% was observed in this cohort. However, there was a deterioration in post-injury performance metrics, including games played per season, plus/minus rating, and time on ice per game post-injury, highlighting the potential devastating sequelae of ATRs in elite NHL athletes.

Achilles tendon rupture (ATR) alters stiffness of the tendon and other structures within the triceps surae muscle–tendon unit. Although stiffness of the tendon has been studied after rupture, regional adaptations of the medial gastrocnemius (MG) muscle and aponeurosis mechanical properties are unknown. Therefore, we assessed changes in MG muscle and aponeurosis shear wave (SW) velocity and morphology during a 1-year follow-up after unilateral ATR. Twenty-three (17 males, 6 females) participants were assessed for SW velocity of MG muscle and aponeurosis and morphological properties at 2, 6 and 12 months at rest. Linear mixed models were used to investigate the differences between limbs at different time points, and partial correlations controlled for age to explore associations between SW velocity and morphological properties. Regional SW stiffness of the injured MG muscle and aponeurosis were lower at 2 months but recovered by 6 months after ATR. When comparing limbs, MG muscle and aponeurosis SW velocity were lower in the injured limb at 2 months with a mean difference of −0.34 m/s (−0.48 to −0.21 m/s, t = -5.10), and −1.6 m/s (−2.39 to 0.89 m/s, t = 4.38). SW velocity did not differ at the muscle or aponeurosis between limbs at 6 or 12 months. Fascicle length of the MG muscle was negatively correlated with SW velocity of the MG muscle (r = -0.25, p = 0.041) and positively correlated with aponeurosis SW velocity (r = 0.29, p = 0.018). The remodelling of the MG muscle to shorter fascicles might help to enhance stiffness and maintain tension at the muscle.

Lower extremity tendon repair can be associated with postoperative pain due to tissue adhesions, fibrosis, and inflammation. Human placental allograft demonstrates anti-inflammatory, antimicrobial, antiadhesive, and antifibrotic properties, but data on its use in orthopaedic surgery is limited. This study compares postoperative pain scores in patients undergoing Achilles tendon repair with adjunctive allograft compared with those without. All patients who underwent open Achilles tendon repair from January 1, 2022 to October 31, 2023 were included in this retrospective study. Numerical pain scores (0 to 10) were obtained at preoperative, 2-week and 4-week postoperative clinic visits. Mean pain scores of patients with and without an allograft were compared. Of 26 total patients, 16 (61.54%) underwent repair with allograft. In this group, the mean preoperative pain score was 3.25. This group showed a much larger and statistically significant decrease in pain to 0.75 (P=0.005) at 2 weeks postop compared with those who did not receive allograft (from 3.00 to 2.60). There is limited data to support the regular use of human placental allograft in orthopaedic foot and ankle surgery. Future studies are necessary to evaluate a larger cohort of patients with longer follow-up, but this study suggests favorable outcomes with the use of an adjunctive human placental allograft.

The objective of this retrospective descriptive study was to characterize accepted compensation claims filed after treatment for acute Achilles tendon rupture (ATR) in Norway, and to explore whether claim outcomes were associated with treatment modality or institutional catchment area.
Methods:
All claims filed to the Norwegian System of Patient Injury Compensation (NPE) after treatment of ATR between 2010 and 2020 were collected and categorized. The claims were organised based on whether they were accepted or denied, the initial treatment given, patient demographics, the reasons given by patients for filing claims, and the rationale provided by the NPE for accepting or rejecting compensation claims. Additionally, hospital patient catchment population were analyzed in relation to accepted claims.
Results:
One hundred forty-six compensation claims were received, of which 61 (41.8%) were accepted. Most accepted claims were related to surgical treatment (n = 30, 49%) or insufficient treatment (n = 22, 36%). The most frequent reason for claim acceptance was delayed diagnosis and/or treatment (15/61, 25%), followed by postoperative infections (10/61, 16%). There was no statistically significant correlation between accepted claims and institutional catchment area population.
Conclusion:
Delayed or missed diagnosis was the most common reason for accepted compensation claims following treatment for ATR. The study found no correlation between accepted claims and the catchment area population of the institution delivering the treatment.

Background: Achilles tendon ruptures pose a challenging recovery for patients, and complications after surgical repair are often
associated with poor patient outcomes. The optimal timing for surgery remains a topic of debate and has not been extensively
studied.
Purpose: To determine whether the time from Achilles rupture injury to surgical repair is associated with postoperative compli-
cation rates and long-term patient-reported outcomes (PROs).
Study Design: Cohort study; Level of evidence, 3.
Methods: Patients undergoing surgical treatment for an Achilles rupture between 2016 and 2022 were retrospectively reviewed.
Patients were stratified based on time-to-surgery (acute: 0-6 days, subacute: 7-13 days, delayed: 14-41 days, and chronic: 42+
days) and operative technique (open vs percutaneous). Surgical complications were assessed using clinical notes, while
PROs—including Patient-Reported Outcomes Measurement Information System Physical Function (PROMIS PF, PROMIS Pain
Interference (PI), and Foot and Ankle Single Assessment Numeric Evaluation (FA SANE)—were collected via a digital survey. A
minimum clinical follow-up of 3 months was required for inclusion in the complication analysis, and a minimum survey follow-
up of 6 months was required for inclusion in the PRO analysis.
Results: Complications were assessed in 350 patients: 116 acute (33%), 131 subacute (37%), 78 delayed (22%), and 25 chronic
(7%). A total of 64 complications occurred in 56 patients (16%): 14 minor wound complications (4%), 8 major wound complica-
tions (2.3%), 13 reruptures (3.7%), 15 deep vein thromboses (4.3%), 3 sural nerve injuries (0.9%), and 9 cases of neuropathic pain
(2.6%). There was no clinically meaningful difference in complication rates among the time-to-surgery cohorts. PROs were avail-
able for 146 patients (42%), with a mean follow-up of 23.8 6 15.6 months and a similar distribution of time to surgery (33% acute,
44% subacute, 17% delayed, and 6% chronic). The mean postoperative PROMIS PF scores were highest in the acute group and
decreased in later time-to-surgery groups (P = .016). No significant differences were found between groups with respect to
PROMIS PI or FA SANE scores.
Conclusion: The timing of surgical intervention after an Achilles tendon rupture did not significantly affect the overall postoper-
ative complication rate. However, patients treated within the first week reported higher PF scores at the follow-up

Objective: To compare the immediate effects of two manual therapy techniques - Compressive Myofascial Release (CMR) and Talocrural Joint Mobilization (TJM) - on passive mechanical properties, ankle mobility, and functional performance in individuals following Achilles tendon repair.

Methods: Twenty participants with surgically repaired Achilles tendons received a single session of either CMR or TJM. Passive mechanical properties (tone, elasticity, stiffness) of the medial/lateral gastrocnemius and Achilles tendon were assessed via handheld myotonometer. Functional outcomes included the Weight-Bearing Lunge Test (WBLT), Achilles Tendon Resting Angle (ATRA), and heel-rise height. Patient-reported outcomes were the Achilles Tendon Total Rupture Score (ATRS) and Foot and Ankle Outcome Score (FAOS).

Results: Demographic variables were similar between groups (p > .05). The mean time since surgery was 12.8 ± 10.3 months in the TJM group and 16.8 ± 13.25 months in the CMR group. CMR led to significantly greater improvements in MG stiffness (p = .007, r = 0.85), MG tone (p = .032, r = 0.68), and LG stiffness (p = .005, r = 0.89), with changes exceeding minimal detectable change (MDC) values. Although Achilles tendon stiffness also decreased significantly (p = .037, r = 0.66), this change did not exceed its MDC. Both groups showed statistically and clinically meaningful gains in ATRA, heel-rise height, and WBLT (all p < .01, r > 0.85). ATRS and FAOS indicated high functional recovery.

Conclusion: CMR yielded greater short-term improvements in clinically relevant muscle-tendon mechanical properties than TJM and may be a valuable therapeutic option following Achilles tendon repair.

A New Surgical Technique for Achilles Tendon Rupture: A Retrospective Study of 18 Cases

Background: The optimal treatment for acute closed Achilles tendon rupture (ATR) is still controversial. Many grassroots hospitals lack advanced equipment to perform effective minimally invasive surgeries, and open surgeries increase the incidence of complications. Therefore, we have developed a new technique specifically for grassroots hospitals. It involves using oval forceps-assisted minimally invasive suture bridge technology to treat acute closed Achilles tendon ruptures. This technique is easy to perform and has shown good clinical outcomes.

Methods: We reviewed the therapeutic effectiveness of using oval forceps-assisted minimally invasive suture bridge technique in the treatment of 18 patients with ATR. All patients were followed up for a minimum of 12 months. Preoperative and postoperative follow-up data were reviewed to analyze the results and evaluate the surgical outcomes.

Results: The patients were followed up for 12-16 months [( 13.9±1.6
) months]. There was no statistically significant difference in the ankle joint visual analog scale (VAS) scores at 3, 6, and 12 months postoperatively ( P> 0.05). However, there was a statistically significant increase in ankle joint active range of motion(AROM) at 3, 6, and 12 months postoperatively ( P< 0.05). At the same time, there was no statistically significant difference in AROM between the healthy side and the affected side at 12 months postoperatively. The American Orthopaedic Foot & Ankle Society(AOFAS) scores and Achilles Tendon Total Rupture Score (ATRS) showed a gradual improvement at 3, 6, and 12 months postoperatively, with statistical significance ( P<

0.05). All surgical incisions healed well without complications such as injury to the sural nerve or incision infection. Three patients experienced pain at the site of ATR at 3 months postoperatively, which resolved after a 2-week course of oral nonsteroidal anti-inflammatory drugs (NSAIDs). One patient experienced transient pain at the Achilles tendon insertion point after 6 months of activity, which disappeared after a 2-week course of oral NSAIDs.

Conclusion: The use of oval forceps-assisted suture bridge technique for the treatment of acute closed ATR is characterized by its small incision, simple operation, and minimal complications, meeting the requirements for early functional rehabilitation of patients. For grassroots hospitals that receive a large number of patients, the oval forceps-assisted suture bridge technique is a surgical method worth popularizing.

Ultrasound-assisted minimally invasive repair of acute Achilles tendon rupture with a shuttling needle

Background: The optimal management of acute Achilles tendon ruptures remains controversial. Open surgical repair (OSR) carries high complication rates, while minimally invasive techniques reduce risks but pose concerns regarding sural nerve injury. This study evaluates the clinical outcomes of ultrasound-assisted minimally invasive repair (MIR) using a double-ended shuttling needle.

Methods: A retrospective cohort of 41 patients (2017-2022) was analyzed: 19 underwent OSR (8-10 cm midline incision, Krackow suture), and 22 received MIR (2 cm incision, ultrasound-guided Bunnell suture with shuttling needle). Operative time, scar length, American Orthopaedic Foot & Ankle Society (AOFAS) score, Achilles Tendon Rupture Score (ATRS), calf circumference, and complications were compared. Statistical analyses included independent t-tests, Mann-Whitney U tests, and chi-square tests (α = 0.05).

Results: MIR demonstrated significantly shorter operative time (18.8 ± 3.0 min vs. 55.2 ± 6.5 min; p = 0.010) and smaller scars (2.02 ± 0.39 cm vs. 10.2 ± 1.9 cm; p = 0.005) versus OSR. No significant functional differences existed at 12 months (AOFAS: 90.2 ± 4.3 vs. 88.6 ± 3.8, p = 0.558; ATRS: 90.9 ± 3.3 vs. 89.7 ± 2.6, p = 0.601). Zero complications occurred with MIR, whereas the OSR had 1 superficial infection, 3 sural nerve injuries, and 2 cases of scar adhesion. MIR patients initiated controlled training at 3 months, showing superior trends in activity resumption.

Conclusion: Ultrasound-assisted MIR reduces operative time by 69.9%, minimizes scarring by 80.2%, and eliminates nerve injury risks while achieving functional parity with OSR. Long-term biomechanical performance and resource accessibility warrant further investigation.

A total of 186 patients (121 males, 65 females; mean age: 47.5 ± 14.6 years) were included. Males were significantly more likely to undergo surgery for acute Achilles tendon repair (93 males, 5 females; P<.0001). Females were more likely to undergo surgery for chronic Achilles tendon pathology (76%). There were no significant sex-based differences in complications or return to activity. There were variations in ethnicity, with Asian patients having a higher likelihood of acute tendon repair (87%) whereas White patients were more likely to undergo surgical intervention for chronic tendinopathy (89%; P<.00001).
Conclusion
Males were significantly more likely to have operative intervention for acute Achilles rupture, while females more often underwent surgery for chronic tendinopathy. There were ethnic differences in presentation patterns. Complication rate postoperatively and return to activity following surgical intervention did not vary between sex or ethnicity.

Acute Achilles tendon rupture (AATR) is a prevalent injury that significantly impacts clinical decision-
making, particularly concerning the initial gap size following the rupture. This review aims to evaluate the
influence of initial gap size on treatment decisions in managing acute Achilles tendon complete ruptures.
A comprehensive search was conducted across four electronic databases - PubMed, Scopus, Web of Science
(WoS), and the Cochrane Library - as well as Google Scholar. Two independent reviewers assessed the
methodological quality of each included study using the Newcastle-Ottawa Scale (NOS), with studies
classified as poor (0-3 stars), fair (4-6 stars), or good (7-9 stars). Discrepancies between reviewers were
resolved by the senior author. Initially, 531 articles were identified; after removing 183 duplicates, 348
articles remained for title and abstract screening. From these, 305 were excluded, resulting in 43 studies
selected for full-text assessment. Ultimately, eight studies, encompassing a total of 679 patients, met the
specified inclusion and exclusion criteria and were included in the final synthesis. The Preferred Reporting
Items for Systematic Reviews and Meta-Analyses (PRISMA) flow diagram illustrates the study selection
process.
Eight studies, encompassing 679 patients, were included. Gap-based treatment protocols were commonly
employed, with a threshold of 5-10 mm influencing surgical decision-making. Re-rupture rates were
consistently low across both operative and nonoperative groups, with no statistically significant differences
reported in most studies. While several studies found no correlation between gap size and the Achilles
tendon Total Rupture Score (ATRS), others demonstrated that gaps >5 mm or >10 mm were associated with
significantly worse functional outcomes or plantarflexion strength deficits. One study highlighted that the
location of the rupture relative to the calcaneal enthesis may better predict long-term strength and fatigue
scores than gap size alone. The quality of the included studies varied, with six rated as fair quality and two as
good quality; none were classified as poor quality.
Larger initial gaps in AATR (especially >5-10 mm) tend to be associated with marginally poorer functional
outcomes and a higher risk of re-rupture, though evidence is mixed. Many clinicians, therefore, use a
conservative cut-off (<10 mm) when selecting nonoperative treatment. However, patients with large gaps
may still do well with modern functional protocols. In decision-making, the tendon gap should be considered
alongside patient activity level and preferences. The literature is limited by small cohorts and variable
methods; future trials are needed to define optimal gap-based guidelines.

Study quality was assessed using the Newcastle-Ottawa Scale. Odds ratios (OR) and 95% confidence intervals were calculated for potential risk factors, and STATA 17.1 was used for analysis.

Results: Nineteen studies were included. Smoking (OR = 2.39), diabetes (OR = 2.18), open wounds (OR = 11.95), and season (autumn and winter) of injury (OR = 3.00) were identified as significant risk factors for SSIs. Introduction
The Achilles tendon is most susceptible to rupture in the lower extremity during exercise. For patients with high requirements for Achilles tendon function, surgery is the preferred method. However, surgical site infections (SSIs) significantly affect functional prognosis and increase the medical burden. This meta-analysis identified risk factors for SSIs in patients with Achilles tendon rupture (ATR).
Methods
We systematically searched Embase, PubMed, Cochrane Library, CNKI (China National Knowledge Infrastructure), Web of Science, VIP (VIP Database for Chinese Technical Periodicals), Wanfang Data, and SinoMed up to October 2024 for studies comparing SSI and non-SSI patients with ATR. A 27-year-old healthy man presented to clinic following a traumatic ski injury with medial ankle swelling and abnormal Thompson test. Magnetic resonance imaging confirmed concomitant Achilles tendon midsubstance rupture and posterior tibial tendon (PTT) dislocation. He underwent surgical intervention of both injuries, including Achilles tendon repair and PTT reduction with flexor retinaculum repair. The patient had an uncomplicated recovery and returned to sports at 1 year postoperatively.
Conclusion:

This is only the second published instance of simultaneous Achilles rupture and PTT dislocation. Awareness of such rare combinations is crucial in guiding workup, accurate diagnosis, and prompt management.

Background: Achilles tendon rupture is a common injury requiring surgical intervention. The choice between Minimally Invasive Surgery (MIS) and Open Surgery (OS) has been widely debated. This umbrella review synthesized the results of systematic reviews and meta-analyses comparing outcomes of MIS and OS for acute Achilles tendon rupture. Methods: A comprehensive search was conducted using PubMed, Embase, Cochrane Library, Web of Science, and Scopus. The AMSTAR-2 checklist was employed to assess the quality of the included systematic reviews and meta-analyses. Data on complication rates, surgical times, functional outcomes, and other patient-centric metrics were extracted and analyzed. Results: An aggregate of 6,480 participants were drawn from 7 included studies (not de-duplicated across overlapping trials). The primary outcomes were re-rupture and validated functional recovery scores; key complications included infection and sural nerve injury; secondary endpoints included operative time and return-to-sport. Searches, selection, and extraction followed prespecified criteria. Because the unit of analysis was published evidence syntheses and the underlying randomized trials overlapped with heterogeneous outcome definitions, results were synthesized qualitatively rather than pooled quantitatively. Across reviews, minimally invasive and open repair showed broadly comparable clinical effectiveness, with differences contingent on technique, perioperative protocols, and follow-up windows. Conclusion: MIS appears to offer significant advantages over OS for the repair of acute Achilles tendon ruptures, including reduced complication rates and faster recovery times, without compromising the effectiveness of the repair in preventing re-ruptures. However, the potential for nerve injury with MIS warrants careful consideration. Decisions regarding surgical techniques should be tailored to individual patient circumstances and the specific expertise of the surgical team.

Percutaneous repair is a safe and reliable method to restore continuity after acute Achilles tendon ruptures, with a lower incidence of wound complications than to open techniques. We describe a percutaneous ultrasound-guided surgical technique for acute Achilles tendon ruptures. Ultrasound guidance enables the tendon to be sutured without damaging the sural nerve or vessels. Following a cadaveric study in seven specimens, the technique was then used in 11 Achilles tendon tear patients. Ultrasound-guided repair of Achilles tendon ruptures is safe and reliable, and does not require skin incisions or a tourniquet. No re-ruptures nor sural nerve damage were recorded at the latest follow-up. Appropriately powered clinical studies are necessary to compare this closed technique with other percutaneous or minimally invasive techniques.

Objectives:
Achilles tendon ruptures (ATR) are critical injuries that substantially impair the performance of athletes, particularly those involved in sports requiring explosive movements and rapid directional changes, such as soccer. These injuries can signficantly impair mobility and performance and are often caused by overstretching or forceful contractions. This study aims to assess the long-term impact of ATR on return to play (RTP) and subsequent player performance across various positions in Major League Soccer (MLS) over a 10-year period.
Methods:
This retrospective matched-cohort study evaluated MLS players who sustained ATR between 2010 and 2021. Injured players were matched to uninjured controls in a 1:2 ratio. Performance metrics, including games played, minutes played, goals, assists, and other key statistics, were extracted from MLS databases and compared between injured players and matched uninjured controls. Statistical analyses were performed using t-tests to determine the impact of ATR on RTP and post-injury performance outcomes.
Results:
A total of 185 MLS players who sustained ATR were identified and compared to matched controls over a three-year period post-injury. Forwards with ATR mostly had comparable outcomes, but showed a significant decline in goals per season three years post-injury compared to controls (10.75 uninjured ; 5.00 injured , p=0.044). Midfielders exhibited no significant differences in performance metrics versus their controls. Defenders demonstrated a significant reduction in goals per season during the index year compared to controls (1.08 ; 0.62 , p=0.009), highlighting an immediate impact on their scoring ability. Goalkeepers displayed a statistically significant increase in clean sheets during the index year compared to controls (8.38 ; 8.75 , p=0.016).
Conclusions:
ATR has a distinct and position-specific impact on RTP and long-term performance in MLS players. Forwards experience a notable reduction in scoring ability three years post injury (-5.75), while defenders demonstrate an immediate decline in goals during the injury year (-0.46). Interestingly, goalkeepers show an increase in clean sheets during the injury year (+0.37), suggesting a differential impact based on position. These findings underscore the necessity for tailored rehabilitation and RTP strategies that address the unique demands of each playing position, facilitating optimal recovery and sustained performance post-injury.

Derivation and Validation of a Risk Calculator for the Prediction of Incidence of Complications Following Repair of Achilles Tendon Rupture

Objectives:
The Achilles tendon is the body’s strongest and largest tendon. It is commonly injured, particularly among athletes, accounting for a significant portion of serious tendon injuries. Several factors play a precipitating role in increasing the risk of these injuries. Our objective is to derive and validate a risk calculator for the prediction of incidence of any complication following Achilles tendon repair.
Methods:
We used de-identified data from the American College of Surgeons’ National Surgical Quality Improvement Project (NSQIP) database from 2005 to 2021. It comprises 7010 individuals who had undergone Achilles tendon rupture repair. Demographic and risk factors information was collected. To develop the calculator, the sample was divided into a derivation cohort (40%) and a validation cohort (60%). Multivariate logistic regression was used for statistical analysis, and a risk calculator for incidence of any complication was derived from the derivation cohort and validated on the remaining 60% of the sample. Patients with missing data were excluded, and the significance level was set at p < 0.05. Based on the adjusted model a risk calculator on the incidence of any complications was computed on Microsoft Excel.
Results:
We analyzed the derivation cohort of 2245 individuals who underwent Achilles tendon repair surgery between 2005 and 2021, with a 5.5% overall complication. Multivariate logistic regression identified anesthesia type, ASA classification, certain co-morbidities (pre-operative dialysis and medication-requiring hypertension), and wound classification as significant predictors of complications (Table 1). The developed risk calculator model had an area under the curve (AUC) of 0.685 in the derivation cohort and 0.655 in the validation cohort, surpassing the widely used and validated modified frailty index with an AUC of 0.601 (Mfi-5) (Figure 1). A cut-off score threshold of 0.06 was established using Youden’s index to dichotomize individuals into low and high risk for developing any postoperative complications (Figure 2).
Conclusions:
In conclusion, our study explored the risks involved in the repair of ruptured Achilles tendons over a 16-year period and assessed the incidence of any complications, as well as emphasizing the mechanisms behind their significance. We highlighted that chronic Steroid use, wound classification, ASA classification, anesthesia type, preoperative dialysis, and HTN requiring medication all played a substantial role in the risk of post-operative complications. This study not only illustrates the complex nature of Achilles tendon repair issues, but it also allowed for the development of a novel and practical risk calculator that may assist patients having this intervention in identifying and managing their risks. This has the potential to improve patient outcomes and guide therapeutic decision-making in the realm of Achilles tendon repair.

Objectives:
Achilles tendon ruptures are one of the most common types of tendon ruptures, and over the decades have increased in prevalence, a phenomenon credited to increasing levels of athletic activities in adults.1,2 Heavy loading and high-speed eccentric muscle action, such as sudden acceleration in running or jumping, put the Achilles tendon under stress and can lead to rupture. Sports and athletic activities are the cause of 68-80% of all Achilles tendon ruptures.2,3 Pickleball is regarded as America’s fastest growing sport, with participation in the sport doubling and membership in USA Pickleball, the sport’s national governing body, increasing by 30% in 2022.4,5 This study aims to examine the functional outcomes of Achilles tendon ruptures sustained while playing Pickleball and treated through surgical intervention.
Methods:
A retrospective chart review of Achilles tendon ruptures was conducted at a single institution. Those patients who sustained an Achilles tendon rupture injury while playing pickleball and had a minimum of one-year follow-up were included. Dates of surgery were between January 2016 and August 2023. Data reviewed were demographics, comorbidities, surgical technique, and complications. Review of patient reported outcome measures (PROMs) included Veterans-Rand 12 (VR-12) General Health Survey, Foot and Ankle Ability Measure (FAAM), Visual Analog Scale (VAS) scores, patient satisfaction and a return to play survey.
Results:
Of 1,968 patient charts reviewed, 127 patients were identified and used for final analysis. The average age was 54.3 years old (SD 11.5) are 77% (98) were males. The average Body Mass Index was 27.0 and 91 were not tobacco users, 10 were current, 22 were former and 4 use marijuana. The average follow-up time was 28.3 months (12-73.7). All PROMs were statistically significant except VR-12 Mental, see table 1. At the time of this report, the average return to play was 18.4 months and 50% have returned to pre-injury play. The differences between pre-operative and post-operative results of PROMs were analyzed with sample t-tests, with a P<0.05 to determine significance.
There were 11 complications noted: 4 minor [wound (2), persistent swelling at 5 months (1) and worsening pain (1)], 3 irrigation and debridement surgeries after wound infections, one deep vein thrombosis that resolved with anticoagulation and 3 re-ruptures, 2 were treated surgically at an average of 3.7 months after the initial date of repair.
Conclusions:
Over a 7.5-year time frame, 6.5% of Achilles Tendon ruptures were Pickleball related. 97.7% of patients were satisfied with their Achilles repair, and the majority returned to play about one and a half years after the injury. Continued follow-up with these patients is warranted to further validate these findings.

Minimum 5-Year Outcomes After Minimally Invasive Achilles Tendon Reconstruction Using Autologous Hamstring Grafts

Background: The Achilles tendon is prone to rupture, particularly in middle-aged men, and chronic injuries are often due to missed diagnoses or inadequate treatment. While acute ruptures can be managed with primary repair, chronic cases with tendon gaps >3 cm require reconstruction. A minimally invasive technique using autologous semitendinosus and gracilis tendon grafts has previously shown favorable short-term outcomes. This study evaluates longer-term outcomes with a minimum 5-year follow-up.

Methods: This study analyzed 21 patients undergoing minimally invasive Achilles tendon reconstruction with autologous hamstring grafts, compared with 21 healthy controls. magnetic resonance imaging confirmed diagnoses, and surgeries (2016-2019) used the EndoButton system for graft stabilization. Assessments included Achilles tendon Total Rupture Score (ATRS) for patient-reported outcomes, EuroQoL-5 Dimensions, 5 Levels (EQ-5D-5L) for overall health, and visual analog scale (VAS) for pain and satisfaction. Functional tests measured dorsiflexion (lunge test), calf endurance (heel rise test), and muscle strength (single-leg hop test), comparing operated vs unoperated legs.

Results: ATRS and EQ-5D-5L Index improved from 2 to 5 years (P = .0136, P = .0396), although significance was lost after Bonferroni correction. Functional test results remained stable. The operated limb achieved >90% of the nonoperated side across tests. Compared with controls, patients reported lower EQ-5D-5L scores and greater pain (P < .05), but satisfaction remained high (VAS score 9/10). One retear occurred due to infection.

Conclusion: Minimally invasive Achilles tendon reconstruction using hamstring autografts results in sustained functional and subjective improvements at a minimum of 5 years postoperatively. Despite modest residual deficits compared with uninjured controls, high satisfaction and strong limb symmetry support the long-term efficacy of this approach. This study demonstrates that the functional and subjective results of patients after Achilles tendon reconstruction with hamstring graft remain stable and satisfactory at a minimum of 5 years postoperatively.

Epidemiology of Acute Primary Tendon Ruptures Presenting to Emergency Departments in the United States

There is literature on epidemiologic trends in acute primary tendon ruptures. However, most of the data used in the literature is centered in Europe or Canada, whereas there has been little analysis of trends in primary tendon ruptures in the United States (US), especially data collected after 2010.
Purpose:
To report the epidemiology of upper and lower extremity acute primary tendon ruptures presenting to emergency departments (ED) in the US and compare incidence ratios based on race using the National Electronic Injury Surveillance System (NEISS) Database.
Study Design:
Descriptive epidemiology study.
Methods:
The NEISS database was queried for acute primary tendon ruptures in the US from 2001 to 2020. National estimates (N) and estimated incidence ratios (IR) reported as per 100,000 person-years at risk (PYR) for acute primary tendon ruptures were calculated using NEISS sample estimates (n) and US Census Bureau population estimates. Temporal trends in annual IRs throughout the study period were assessed with regression analyses.
Results:
From 2001 to 2020, there were an N of 235,189 (95% CI, 186,833-283,546) ED visits for acute tendon ruptures in the US, which occurred in a population of 6,183,899,410 person-years for an overall IR of 3.80 PYR (95% CI, 3.02-4.59). Acute primary Achilles (38.2%), biceps brachii (16.2%), and patellar tendon (12.7%) ruptures were the most common injuries. Black patients had the highest overall IR, with an N of 38,679 ruptures (95% CI, 21,552-55,807) occurring in this group for an overall IR of 4.78 PYR (95% CI, 2.66-6.89). The IR of lower extremity tendon ruptures was significantly higher in Black patients, but did not differ for upper extremity tendon ruptures. The annual incidence of all tendon ruptures in the US increased significantly from 2001 to 2020, with an average annual percent of 3 (95% CI, 2.3-3.7; P < .0001).
Conclusion:
Our study demonstrated that the IR of acute primary tendon ruptures presenting to the ED in the US has increased from 2001 to 2020. Black/African-American individuals have an increased IR compared with White populations.

Knotted or knotless? Efficacy analysis of the percutaneous Achilles repair system (PARS) for acute Achilles tendon ruptures: a retrospective cohort study

Purpose: The Percutaneous Achilles Repair System (PARS) has been applied in patients with acute Achilles tendon ruptures, encompassing both knotted and knotless methods. Whether these two approaches have different impacts on patient outcomes remains controversial. This study aims to investigate the clinical efficacy of these two surgical techniques in the treatment of acute Achilles tendon ruptures.

Methods: We retrospectively collected data from 61 patients with acute Achilles tendon ruptures treated at our center between January 2020 and January 2024. Patients treated with the knotless technique were classified as the experimental group, while those treated with the knotted technique were classified as the control group. Comparisons were made between the two groups regarding the American Orthopaedic Foot and Ankle Society Score (AOFAS) and Achilles Tendon Total Rupture Score (ATRS), surgical time, hospital stay duration, and hospitalization costs. Statistical analysis was conducted using the Student's t-test and the Chi-square test to compare differences between groups.

Results: The results of this study suggest that both techniques under the PARS are effective for patients with acute Achilles tendon rupture. Additionally, there were no statistically significant differences between the two groups in terms of the AOFAS, ATRS, surgery time, hospital stay, or hospitalization costs (P > 0.05). Notably, two patients in the experimental group experienced varying degrees of postoperative discomfort, whereas no complications were observed in the control group.

Conclusions: The application of the PARS is an effective treatment for Achilles tendon ruptures. However, we observed certain specific complications associated with the knotless technique, such as anchor irritation and irritation caused by anchor sutures in areas with thin skin.

Intraoperative Application of Hyaluronic Acid in Achilles Tendon Repair: A Retrospective Cohort Study on Short-Term Functional Outcomes

Background and Objectives: This study aimed to investigate the effect of intraoperative hyaluronic acid (HA) application on short-term functional outcomes after open surgical repair of Achilles tendon rupture. Materials and Methods: This retrospective cohort study included 102 patients screened at three tertiary centers (January 2023-October 2024). After applying the exclusion criteria, 64 patients were analyzed (32 HA, 32 control), with group allocation based on the intraoperative use of HA. The primary outcomes were the American Orthopaedic Foot and Ankle Society (AOFAS) Score and the Achilles Tendon Total Rupture Score (ATRS), evaluated at 3, 6, and 12 months; the Short Form 36 (SF-36) Physical Component Summary (PCS) and Mental Component Summary (MCS) scores, assessed at 6 and 12 months; and physical-performance tests (heel-rise endurance, single-leg hop distance, and calf muscle strength), performed at 6 and 12 months. Complications, including re-rupture, wound infection, and delayed wound healing, were also recorded. Results: At 6 months, the HA group had higher AOFAS scores (mean difference: 9.0, 95% CI: 6.3-11.7, p = 0.008) and ATRSs (mean difference: 7.0, 95% CI: 3.8-10.2, p = 0.008) than the controls. The differences were smaller but remained significant at 12 months (AOFAS mean difference: 5.0, 95% CI: 2.5-7.5, p = 0.034; ATRS mean difference: 4.0, 95% CI: 1.1-6.9, p = 0.034). The SF-36 PCS and MCS scores were also superior in the HA group at 6 and 12 months (all p < 0.05). The physical-performance tests (heel-rise endurance, single-leg hop, calf strength) showed significant mean differences, with the HA group scoring higher at both timepoints (all p < 0.05). Complication rates were low and not significantly different between groups (all p > 0.05). Conclusions: Intraoperative HA application during open Achilles tendon repair was associated with improvements in short-term functional recovery, general health status, and physical performance without increasing complication rates. These findings support the use of HA as a safe adjunct to optimize tendon gliding; however, prospective randomized controlled trials are needed to confirm these results.

Successful functional outcomes and return to sport rate can be achieved after surgery for acute Achilles tendon rupture: A systematic review

Purpose: Achilles tendon disorders are frequently seen in sports, and its rupture is one of the most common and debilitating injury. Among the most used surgical techniques there are traditional open surgeries, minimally invasive, and percutaneous techniques. The choice of technique often depends on the nature of the injury, the athlete's profile, and the surgeon's preferences. This systematic review aimed to analyze the functional outcomes, return to sport (RTS) rate and time, and complications in patients who underwent surgical repair for acute Achilles tendon lesions.

Methods: The PubMed, MEDLINE, Scopus, and Cochrane Central databases were used for the research, and 9 studies were included. The first author, journal name, year of publication, patient demographics, type of sports, level of play, dominant limb and follow-up period were recorded for each article. Data extracted for quantitative analysis included different types of lesions, types of surgical repair, RTS rate and time, the visual analog scale (VAS) for pain, the AOFAS score, the Tegner score, and the ATRS score, and the number and types of complications.

Results: A total of 748 patients who underwent surgical repair of Achilles tendon were identified. Male patients represented 84% of the cases. The frequency-weighted mean age at the time of the operation was 40.7 ± 11.8 years, and the frequency-weighted mean follow-up was 40.9 ± 11.7 months. The postoperative functional outcomes improved. A total of 579 patients (77.4%) returned to sport. Postoperative infection was reported in 25 patients (3.3%) and Achilles tendon re-rupture was reported in 17 patients (2.3%).

Conclusions: Patients who underwent Achilles tendon surgical repair reported successful functional outcomes and low postoperative pain scores after a mean 3.5-year follow-up. Postoperative AOFAS and Tegner scores higher than the normative values can be achieved. The RTS rate was 77% with a mean time of 8.1 months. Postoperative infection and tendon re-rupture were the most common reported complications.

The patients were categorised into two groups (accelerated rehabilitation group, AR, and conventional rehabilitation group, CR) according to whether postoperative rehabilitation relied on US monitoring. All patients received the same suture technique and immobilisation duration; they were clinically examined at 2, 4, 6, 8, 10, 12, 14, and 16 weeks postoperatively, with a final follow-up at a mean of 18.8 months. The primary outcome was the recovery time for the one-leg heel-rise height (OHRH). Secondary outcomes included the time required to return to range of motion(ROM) and light exercise (LE), the clinical function scores, and complications. Data regarding the surgical duration, the visual analogue scale (VAS) score for pain, the Achilles tendon Total Rupture Score (ATRS), and the American Orthopaedic Foot and Ankle Society (AOFAS) Ankle-Hindfoot Scale score were also collected.
Results

The recovery times for OHRH and LE were significantly shorter in group AR than in group CR (P < 0.05). The VAS scores decreased over time, reaching 0 in both groups by 10 weeks. ATRS and the AOFAS Ankle-Hindfoot scale score increased across both groups over time, showing significant between-group differences from 4 to 14 weeks(P < 0.05) and 4 to 8 weeks(P < 0.05). The mean scores were better in group AR than in group CR. Three re-ruptures (3.8%) were observed, and all complications were resolved at the last follow-up, with no significant between-group differences.
Conclusions

Rehabilitation under Ultrasonography monitoring safely accelerates recovery after acute Achilles tendon repair, improving short-term outcomes without increasing pain or re-rupture risk.
