Anyone using the mini-tight rope/bunionectomy procedure?

    One Idea that a college and I have discussed is to use a 3.5 or 4.0 screw to stabilize the 1st TMTJ at it's base to the 2nd met. Somewhat of a base wedge variant with the TW. Idea being the added stability would take pressure from the 2nd met, avoiding stress frx.

    I understand the 1-2 screw has been tried before with less than stellar results.

    I have done probably close to 75 bunion procedures utilizing the mini-tight rope procedure. The procedure has evolved in the past 2 years. The most current implant utilizes a 1.1 K-Wire to drill the holes in the second metatarsal. This has significantly cut done on the incidence of 2nd metatarsal stress fractures.

    There is significantly less disability with this procedure than any osteotomy alternative bunionectomies. My patients, overall are very happy with this alternative to traditional bunion surgery.

    Do you use this for anything more than a 15 deg IMA?

    Previous colleague performed them, resulted in 2nd met fractures too often-
