cold exposure

    A new patient, 58 Y/O poorly-controlled DM, seen through ED for consult. Six weeks ago, he was exposed to high concentrations of dry ice, resulted in present-day full thickness dry gangrene to all digits, and plantar heel ulcers as well. I debrided, did full C&S, bloodwork, Endocrine referral, have home healthcare doing woundcare with aquacell Ag, expecting autoamputation in the near future...Gen Surgeon wants to do trans-met amputations....patient wants to wait and allow demarcation. Thoughts? Anything missed?

    would allow demarcation to determine amp level.Unless threatening infection . Some tissue may even be regenerated.
