Diabetic Evaluation form

    Hello everyone,

    I was wondering if there was a form similar to the foot posture index but for a diabetic foot evaluation.
    At the end of the evaluation I would like a score or risk factor rating.

    Is there one? I made mine up myself, and just gave the patients a rating low (medium) and high.

    High being previous ulcer or amputation, or compromised vascular status.
    Medium technically doesn't exist or so I understand from the Diabetic Foot Conf, but I use it if there are neuropathies, footwear issues or callus, it allows a bit of room for common sense.
    Low risk being none of the above.

    As I understand it, it is relatively simple system, despite the consequences being severe if you get it wrong.

    (Disclaimer - I am a private practitioner not a diabetic foot specialist)
