Emergency Telephone Triage Form

    Is anyone using a form to triage emergency telephone calls which the reception staff fill in and pass to the podiatrist in clinic? We are trying to eliminate ambiguity, but do not want to re-invent the wheel if someone already has a form that works.

    Drown001, wow you almost missed the welcoming committee, that what happens when you jump if with both feet and miss the introduction forum.
