    Question on questionnaires/patient information/education and do you have samples; verbal, printed and internet:

    How are you are evidencing patient advice and information:

    1. Prior to visit
    2. The Visit
    3. The treatment Programme
    4. Follow-up visit
    5. Aftercare/Education

    Seeking continual feedback from patients:

    1. Undertaking regular surveys on satisfaction and improvements
    2. Evidence how feedback is used

    Any help would be appreciated.
