INTRODUCTION:
This case report will have implications for any area of medicine that aims to redistribute plantar pressure away from a particular area of the foot. This could be for example in the short-term care of people with diabetes, people who have insensate feet and people with poor blood supply to the foot coupled with plantar ulceration. The aim of the study was to investigate which type and thickness of Hapla felt padding is the most effective at redistributing plantar pressure of the foot. This case report is the first of its kind.
CASE PRESENTATION:
The participant was a healthy 50-year-old white man with a high peak plantar pressure over the second metatarsal head of both feet; he required removal of a plantar callus on a periodic basis.
CONCLUSIONS:
The reader should note that different types of Hapla felt padding provide different forms of redistribution of plantar pressure on the foot. In the clinic it may be useful to measure peak plantar pressure using F-Scan before deciding on the most appropriate type of felt padding.

The Applicability of Plantar Padding in Reducing Peak Plantar Pressure in the Forefeet of Healthy Adults

Background: We investigated the effectiveness and durability of two types of plantar padding, the plantar metatarsal pad and the single wing plantar cover, which are commonly used for reducing forefoot plantar pressures.

Methods: Mean peak plantar pressure and impulse at the hallux and at the first, second, third, and fourth metatarsophalangeal joints across both feet were recorded using the two-step method in 18 individuals with normal asymptomatic feet. Plantar paddings were retained for 5 days, and their durability and effectiveness were assessed by repeating the foot plantar measurement at baseline and after 3 and 5 days.

Results: The single wing plantar cover devised from 5-mm felt adhesive padding was effective and durable in reducing peak plantar pressure and impulse at the first metatarsophalangeal joint (P = .001 and P = .015, respectively); however, it was not found to be effective in reducing peak plantar pressure and impulse at the hallux (P = .782 and P = .845, respectively). The plantar metatarsal pad was not effective in reducing plantar forefoot pressure and impulse at the second, third, and fourth metatarsophalangeal joints (P = .310 and P = .174, respectively).

Conclusions: These results imply limited applicability of the single wing plantar cover and the plantar metatarsal pad in reducing hallux pressure and second through fourth metatarsophalangeal joint pressure, respectively. However, the single wing plantar cover remained durable for the 5 days of the trial and was effective in reducing the peak plantar pressure and impulse underneath the first metatarsophalangeal joint.

Effectiveness and Duration of Plantar Pressure Off-loading by Two Designs of Felt Padding: A Preliminary Study.

BACKGROUND:
The aim of this study was to observe the pressure changes in the felt padding used to off-load pressure from the first metatarsal head, the effects obtained by different designs, and the loss of effectiveness over time.

METHOD:
With a study population of 17 persons, two types of 5-mm semicompressed felt padding were tested: one was C-shaped, with an aperture cutout at the first metatarsophalangeal joint, and the other was U-shaped. Pressures on the sole of the foot were evaluated with a platform pressure measurement system at three time points: before fitting the felt padding, immediately afterward, and 3 days later.

RESULTS:
In terms of decreased mean pressure on the first metatarsal, significant differences were obtained in all of the participants (P < .001). For plantar pressures on the central metatarsals, the differences between all states and time points were significant for the C-shaped padding in both feet (P < .001), but with the U-shaped padding the only significant differences were between no padding and padding and at day 3 (P = .01 and P = .02).

CONCLUSIONS:
In healthy individuals, the U-shaped design, with a padding thickness of 5 mm, achieved a more effective and longer-lasting reduction in plantar pressure than the C-shaped design.

The effects of felted foam in diabetic foot treatment: systematic review with meta-analysis

Objective: To evaluate the effects of felted foam in the treatment of diabetic foot plantar ulcer.

Method: Systematic review with meta-analysis. Research was conducted in Cochrane Library, PubMed, CINAHL, Science Direct, Web of Science, SCOPUS, IBECS and LILACS. Cochrane Collaboration Risk of Bias Tool was employed to evaluate risk of bias. The meta-analyses were calculated in the program Review Manager, while Grades of Recommendation, Assessment, Development and Evaluation was used for evaluating evidence quality.

Results: Four clinical essays and two cohorts were included. Mean plantar pressure of individuals exposed to felt was reduced by 10.77 kilopascals (CI 95% -14.92, -6.62; p<0.001).

Conclusion: Higher reduction of plantar pressure was observed to be associated with the use of felt. Development of new models of relief orthosis with felted foam for plantar pressure is recommended, along with more clinical research elucidating related outcomes.

Semi-compressed felt is frequently used in clinical podiatric practice to offload areas of high pressure,
particularly in patients with chronic ulceration. The study aimed to assess the effectiveness of semicompressed felt, on plantar pressures, in offloading the 1
st metatarsophalangeal joint, whilst assessing
pressure encountered at the peripheries. Elevated plantar pressures are a strong predictor of ulceration in
patients with diabetic foot disease, where undetected mechanical trauma can rapidly become ulcerative. In
this study, plantar pressure was assessed using the Foot Work Pro plate (AM CUBE, Inc., France,
www.amcube.net), in three conditions: barefoot; 5mm thickness and 10mm thickness conditions. This
study was of a cross sectional design, where participants were conveniently sampled within the Podiatric
Medicine student population within the National University of Ireland, Galway. 33 participants (28 females;
5 males) with a mean age of 23 years (Interquartile Range, 18-44). Plantar pressures at the 1
st
metatarsophalangeal joint decreased (P<0.01; P<0.01), when semi-compressed felt was applied to offload
the 1
st metatarsophalangeal joint. Whereas, plantar pressures were found to be increased at both the 3
rd
metatarsophalangeal joint (P= 0.04; P= 0.01) and the 5
th metatarsophalangeal joint (P=0.82; P=0.40), as a
result of applying felt to the 1st metatarsophalangeal joint. Evidently, offloading one joint subjected other
joints to greater mechanical load allowing insight into the mechanical redistribution associated with the
use of felt in offloading, which must be accounted for in the high risk diabetic foot.
