Pre - op for nail surgery

    Hi

    I've got a patient who is allergic to Iodine and is having a partial nail avulsion on Wednesday. Can anyone suggest a suitable pre-op please?

    Hi Amanda,

    It`s quite rare for a person have a true allergy to Iodine. See here; http://www.podiatry-arena.com/podiatry-forum/showthread.php?t=84822&highlight=iodine

    That said, I have a few who report `Iodine allergy` so for those I use a skin prep of Chlorex and 70% IPA. http://www.ncbi.nlm.nih.gov/pubmed/19796594?dopt=Abstract

    Partial nail avulsion....
    Just wipe with alcohol. Nails are considered a "dirty" procedure so you cannot really apply true sterile technique to it.
    Our protocol for simple nail avulsions is merely use an alcohol wipe. Patients are soaking the next day anyway which is hardly a sterile environment since they normally submerge their entire foot in the soak. The wound is open.

    The best you can do in not introduce any new pathogens so use sterile instruments and gloves (obviously)

    Another vote for chlorhexidine in alcohol.

    Also agree with Steve. Reminds me of debriding diabetic foot ulcers, when you take the dressings off and find dog hair, cigarette buts, maggots etc, falling out. Of course, use aseptic technique. But it's all relative.

    The worst thing about feet is we tend to walk on them.
