I'll be launching a new practice in the next few months after a 3-year sabbatical.

I want to create two surveys - one for the general public and another for other healthcare practitioners - to understand the needs of the community in the area I'm thinking of setting up shop.

I've already brainstormed some questions for both surveys but would really appreciate your input as to what to include (and what not to).
