Neuroma Injection Impact on Potential Stress Fracture

    38 yo Female with previous 3rd metatarsal stress fracture 08/2022, presents with pain in the 3rd metatarsal on 6/5/2024, with negative tuning fork test. Given complications from previous injury, patient elected to have MRI (X-ray showed no difference between present and 2022 post healed stress fracture).

    MRI was done 2 weeks post initial symptoms and revealed neuroma between 3rd and 4th metatarsal. No stress fracture observed. Injection provided.

    Patient continues to have swelling and redness across the foot, and numbness in her 3rd toe. Pain continues to occur when pressure is applied to 3rd metatarsal bone 3 days post injection. Walking short distances increases swelling and 'hot' feeling.

    Patient is concerned that stress reaction was missed on the MRI, and now with the injection, bone healing and potential for worsening of the stress reaction is possible. Please provide guidance on how long post injection the bone is in jeopardy of greater chances of bone loss as well as what the impact on healing time could be if there is indeed a stress reaction in the 3rd metatarsal.

    Image is of feet 3 days post injection upon waking. Redness reduces some throughout day.

    For further context, patient's original injury in 2022 was complicated -

    7/7: injury
    7/13: seen by podiatrist and told to wear boot for 2 weeks
    7/27: told no stress fracture and to get out of the boot (patient resumed exercise / hiking)
    8/2: patient came back in due to pain and was diagnosed with neuroma between 2nd and 3rd metatarsal, injection given (patient resumed exercise / hiking)
    8/12: patient diagnosed with neuropathy and given injection (patient resumed exercise / hiking)
    8/22: patient asked for mri and stress fracture on 3rd metatarsal revealed
    patient wore boot for ~7 weeks until healed
    (unsure if stress structure was present at time of initial pain 7/7 or developed between 7/27-8/22)
