Orthotic outcome questionnaire

    Hi folks, am currently trying to create a questionare for an audit within the health service I currently work, re: "Patients perceived efficacy/symptom relief, of prescribed orthoses". Has anyone devised anything similar? Or be able to point me in a suitable direction for helpful info.
    Any help much appreciated
