Any suggestions, please, for my patient who wants to start trying for a family and is unwilling to wait until her onychomycosis clears.

    Thanks

    Hi Debbie,

Have you tried Imperial Feet Nail Mycosis solution? - It may/may not help, but it's worth a try.

    Regards

    Hi Anne,

I've never heard of it. Do you have the name of a supplier?

    Thanks

    Hi Debbie,

It's distributed by Canonbury Healthcare Ltd.

Interesting work on topical garlic and the use in onychomycosis.
Stephen Davis is the scientist at the mycology department at the Women's and children who has done alot of work of garlic.
Apparently mash fresh garlic into a poultice mix with alcohol and put on nails every night for a couple of hours.

This is great if you can live with the smell!

I have tried fresh garlic poultices on verrucae and nails. I even treated my son with it at one stage for verucae.

As a family, we love to eat garlic, but the pervasive smell of this throughout the house when applied to my son's feet, would have kept the vampires and anyone else away!

Unfortunately, it did little for the VPs.

    Regards

That is a good query. My advice would be to try a topical such as Carmol 40 as that thins out the nail plate.

    Thanks everyone for your help.

Does anyone know the definitive answer to whether Tea Tree Oil is safe in pregancy? I understood that it was not, but it is contained in the Imperial Feet solution.

I don't suppose garlic does great things to people suffering morning sickness, but is another option.

Again I'm unfamiliar with Carmol 40, but will look it up on the web.

After all the effort, my patient cancelled her appointment, but I'm sure the information will come in handy again some time.

    Best wishes

    Debbie

Carmol 40 is good.It contains urea which is an emollient.I use it a lot and I think it is safe in pregnancy as I am very reluctant to give oral terbenifine or itraconazole and I sure am not doing anything with anesthesia unless cleared by an MD.

Just a note on the garlic (as I'm not sure why pregnancy has to be delayed until you're free of fungal nail infections - perhaps someone can produce the definitive guidelines on that one?)...............you can get a garlic preperation which has been prepared in such a way as to preserve the main active ingredient (allicin) called allimed / allimax.

This comes in sprays, liquid, cream and tablets. The tablets are available from most chemists, but the other preperations are only available over the internet, which you can access at http://www.allimax.com/shop/.

I have tried a combination of the spray (for the nails) and tablets and it has proved successful on those that have been trialled so far, but a lot slower than e.g. terbinafine

    Just a thought!

I have frankly never tried any garlic preps on a patient with onychomycosis but this sounds like a good idea.

    Hi nicpod1,

The preganancy didn't have to be delayed because of the fungal nails. It was that her treatment was going so well, but the treatment was inappropriate during pregnancy. Hence my question.

Thanks for the really useful advice re. garlic preps.

    Hi Debbie

Another approach with this patient might be to try using Tagetes mass followed by the Tincture for fungal nails. I have used this before in pregnancy with reasonable results.
