When should a patient consider having surgery to remove a bone spur on top of instep? Shoes with orthotic insert is beginning to cause rubbing but no real pain. What would your advice to patient be? I know what I advised.

Surgery is indicated to: 1. Relieve pain and 2. improve function.

If you have tried conservative treatments and they have failed then you
are a surgical candidate if you have pain and it interferes with your normal activity.

Not sure what exact anatomic part you refer to as "instep" - you may wish to clarify this for a more definitive opinion.
