Febuxostat Versus Allopurinol Use in Gout Patients: A Real-World Study

Introduction/objectives: The primary goal of this study is the epidemiological observation and characterisation of a gout patient cohort. Following this, allopurinol, febuxostat, or no uric acid-lowering treatment was given, and cardiovascular events and deaths during the follow-up were noted. We precisely documented and studied each patient's comorbidities and co-medications for the analysis.

Method: Our analysis included 208 gout patients, focusing on their epidemiological and clinical features. We monitored the treatment progress of 155 patients using the prescribed medications. Patients received treatment in the rheumatology department at Dresden University Hospital. On average, participants were followed up for 248.7 weeks (4.8 years). We conducted an association study, grouping participants based on their primary uric acid-lowering medication (if they used it for at least 60% of the follow-up period).

Results: In the follow-up, 7.5% of patients in the allopurinol group (five patients) and 18.1% (13 patients) in the febuxostat group experienced at least one cardiovascular event. Among participants primarily using allopurinol, four deaths (6%) were recorded, compared to five deaths (6.9%) among those mainly using febuxostat.

Conclusions: While the febuxostat group showed more events, this was associated with a higher prevalence of almost all risk factors. Interpreting study results and applying them practically remain difficult given the constellation's commonality in clinical life; therefore, careful consideration is necessary.

Efficacy and Safety of Long-term Administration of Various Doses of Colchicine in Patients with Gout

to compare the efficacy and safety of different doses of colchicine (0.5 mg/day vs. 1.0 mg/day) used for the prevention of arthritis attacks in gout patients during the initiation of urate-lowering therapy.

Materials and methods: -The study included 96 patients diagnosed with gout. Patients were randomized into three groups: those receiving colchicine 0.5 mg/day, those receiving colchicine 1 mg/day, and those without anti-inflammatory therapy. All participants were prescribed febuxostat 80 mg/day. The duration of the observation period was 6 months. The frequency and severity of arthritis attacks, as well as the incidence of adverse events, were compared.

Results: -Patients who did not receive colchicine experienced arthritis attacks more frequently compared to those receiving colchicine 0.5 mg/day (p = 0.03) and 1 mg/day (p = 0.007). In the groups receiving colchicine 0.5 and 1.0 mg/day, the frequency of attacks did not differ significantly (p = 0.6), nor did the proportion of patients who did not experience arthritis attacks (18 (56%) and 22 (69%), respectively, p = 0.3). Among the patients not taking colchicine, 9 (28%) did not develop arthritis attacks (p = 0.02 compared to those taking 0.5 mg/day and p = 0.001 for 1 mg/day). The administration of colchicine 1 mg/day (but not 0.5 mg/day) was associated with lower pain intensity on the visual analog scale (VAS) during arthritis attacks compared to the non-therapy group (p = 0.04). The frequency of adverse events was comparable across the groups.

Conclusions: -The use of colchicine 0.5 mg/day for the prevention of arthritis attacks in gout patients is justified due to the absence of differences in efficacy compared to the higher dose. However, prescribing colchicine 1 mg/day also demonstrates a good safety profile and may be considered for the prevention of arthritis attacks.

The 100 top-cited articles in gout: A bibliometric analysis

Background: Numerous studies have investigated gout; however, a focused bibliometric evaluation of the most influential publications remains limited. This study identified and analyzed the top 100 most-cited articles on gout.

Methods: The Web of Science Core Collection Science Citation Index Expanded database was searched using the term "gout." Articles published from January 1, 2000 to April 30, 2026 were screened. The top 100 most-cited articles were selected, and data on citation rank, publication year, journal, impact factor, country, institution, author, study type, research topic, and keywords were extracted. Bibliometric analyses were performed to characterize publication patterns and research trends.

Results: The most-cited article was published in 2006 and received 4359 citations. The top 100 articles were published in 34 journals, with Annals of the Rheumatic Diseases contributing the largest proportion. The United States was the leading contributing country. The University of Pennsylvania was the most productive institution, and Choi HK was the most prolific author. Observational studies were the most common study design. Keyword co-occurrence analysis identified 4 major research domains: epidemiology, pathophysiological mechanisms, pain management, and comorbidities. Frequently occurring keywords included hyperuricemia, gout, management, risk, arthritis, uric acid, prevalence, coronary heart disease, quality of life, and febuxostat.

Conclusion: This bibliometric analysis summarizes the most influential publications in gout research from 2000 to 2026. Major research priorities have shifted from serum urate, diagnosis, and pharmacological treatment toward risk assessment, long-term outcomes, disease burden, and comprehensive management. Hyperuricemia and urate-lowering therapy remain central themes, while emerging topics such as gut microbiota suggest increasing interest in multidisciplinary and precision approaches to gout care.

Caregiver Perspectives on the Burden of Disease and Treatment in Uncontrolled Gout

Objective: Uncontrolled gout (UG) refers to persistently elevated serum urate (SU) levels >6 mg/dL and ongoing gout symptoms despite use of urate-lowering therapy (ULT). The objective of this study was to evaluate the burden associated with informal caregiving for individuals with UG.

Methods: To inform survey development, concept elicitation (CE) interviews were conducted with informal caregivers of patients with UG between April and May 2024. A separate cohort of informal caregivers, meeting the same eligibility criteria, was recruited via physician panels to complete a web-based, cross-sectional survey between January and February 2025. Survey outcomes included caregiver health-related quality of life (HRQOL), productivity, and 12-month patient healthcare resource utilization.

Results: Four caregivers participated in CE interviews, which informed survey development. Separately, 40 caregivers completed the survey. Survey respondents were predominantly female (73%) with a mean (standard deviation [SD]) age of 55.1 (10.6) years, and most (73%) were spouses/partners of patients with UG. UG affected caregivers' HRQOL, with over 30% feeling at least "quite a bit" stressed, overwhelmed, or physically exhausted/drained. Between 36%-50% of caregivers reported at least "moderate" burden associated with assisting patients with UG treatment, depending on the ULT used. Caregivers reported a 36% reduction in their ability to work among those employed and a 41% reduction in their ability to perform daily activities overall. On average, employed caregivers missed 2.7 hours of work in the prior 7 days due to caregiving responsibilities.

Conclusion: Caring for a patient with UG substantially negatively impacted caregivers' HRQOL and interfered with caregivers' ability to work and perform daily activities.

The Evolving Landscape of Gout in the Female: A Narrative Review

Gout is at least three times more prevalent in males than in females. However, concurrent with rising total gout prevalence, complex factors, including comorbidities, diet, lifestyle, and aging, have promoted higher gout prevalence in females. This narrative review focuses on summarizing recent developments in the landscape of gout in females and the mechanisms involved. New knowledge on sex hormone effects on both urate-excreting and urate-reabsorbing transporters and higher hypertension and chronic kidney disease prevalence in females compared to males may help explain why gout incidence rises robustly after menopause in females, to approach that in males. Racial and ethnic factors, risk profiles based on heritable genetic polymorphisms of urate transporters, diet, body mass index, and lifestyle factors differ according to sex. In addition, sex differences in clinical phenotypes, outcomes of gout, and non-gout illnesses include more frequent comorbidities, more pain and disability during gout flares, different perceptions of disease burden, and more frequent severe cutaneous hypersensitivity reaction to allopurinol in females. Collectively, such findings support the potential clinical benefits of tailoring gout and hyperuricemia treatment according to sex.
