BACKGROUND:: Many clinical trials have shown the effectiveness of platelet releasate on chronic wound healing, but large volumes of blood must be aspirated from patients and a platelet separator is required. Moreover, in the case of using homologous platelets, time and effort are needed to locate a suitable donor, and the donor's blood sample must be tested for a history of infectious disease. The current study was undertaken to evaluate the effect of a straightforward method using a blood bank platelet concentrate in treating diabetic foot ulcers.

METHODS:: Fifty-two patients with diabetic foot ulcers were treated using a blood bank platelet concentrate. A control treatment (i.e., treatment with topical fibrinogen and thrombin) was performed on 48 patients. Twelve weeks after treatment, the percentage of complete healing, mean healing time, percentage of wound shrinkage, and patient satisfaction were compared.

RESULTS:: Complete wound healing was achieved in 79 percent of the blood bank platelet concentrate-treated group and 46 percent of the control group (p < 0.05). The times required for complete healing were 7.0 +/- 1.9 and 9.2 +/- 2.2 weeks in the blood bank platelet concentrate-treated and control groups, respectively (p < 0.05). The degrees of wound shrinkage were 96.3 +/- 7.8 and 81.6 +/- 19.7 percent for the treated and control groups, respectively (p < 0.05). Patient satisfaction with the blood bank platelet concentrate treatment yielded better results than the control method (mean score, 7.6 +/- 1.6 and 5.3 +/- 1.4, respectively; p < 0.05). No adverse events related to the study treatment occurred.

CONCLUSIONS:: Treatment of diabetic foot ulcers using a blood bank platelet concentrate showed results superior to control treatment. A blood bank platelet concentrate offers a simple and effective treatment for diabetic foot ulcers.

Non healing diabetic foot ulcers and the resulting potential amputations present significant costs to the health care system and reduce patient quality of life. The goal of diabetic foot ulcer treatment is to obtain wound closure as expeditiously as possible. The use of platelet-rich plasma (PRP) to enhance wound healing has increased dramatically over the last decade. However, controversies exist in the literature regarding the added benefit of this procedure. The aim of this study is to investigate the efficiency of platelet releasate on the healing of chronic diabetic ulcers in comparison with platelet-poor plasma (PPP). This study included 24 patients with chronic diabetic ulcers. They were systematically randomised into two groups: PRP group (n = 12) and PPP group (n = 12). The results showed that healing in PRP group was significantly faster (P < 0·005). PRP enhances healing of chronic diabetic foot ulcers.

How important are mean platelet volume and neutrophil values in diabetic foot amputation decision?

Background
Previous studies have shown mean platelet volume (MPV) levels to be an important biomarker in the formation of foot ulcers in diabetic patients. The aim here was to show whether MPV, monocyte, platelet, and neutrophil levels could guide diabetic foot amputation.

Methods
Two groups of patients diagnosed with type 2 diabetes and homogeneous in terms of age, sex, diabetes duration, body mass index (BMI), and hemoglobulinA1C (HbA1C) levels were included. The first group consisted of 36 patients with amputation, and the second group consisted of 30 patients with ulceration but no indication for amputation. Besides overall demographic characteristics, preoperative HbA1C, MPV, platelet, monocyte, and neutrophil levels were determined in both groups.

Results
The amputated group had higher MPV and neutrophil levels than the non-amputated control group (p = 0.028 and p < 0.001, respectively). Neutrophil levels could differentiate amputations at a rate of 69.44% and non-amputations at 76.67%. Platelet levels were not statistically significant in differentiating between amputated and non-amputated patients (p = 0.064). Neutrophil levels were 1.207 times higher, and platelet levels were 1.005 higher in the amputation group (p < 0.05).

Conclusions
The atherosclerotic process that occurs during the course of diabetes is thought to be associated with high MPV levels in patients with diabetic foot ulcers. However, the findings obtained here support the use of neutrophil and platelet levels as a marker to predict amputation in diabetic foot monitoring. Further prospective studies with larger numbers of patients are needed to determine the role of MPV levels in the follow-up of diabetic foot ulcers.
