Mid foot pain

    wish you all merry Christmas and new year.

    here with x ray of 13 yrs athletic female, has pain over her mid foot pain bothe side started after practicing 100 meter running for last 2 months.now she can't perform
    sports activities.
    she has tenderness over navicular region and got generalised ligament laxity with flat feet.

    acc. navicular./stress # of navicle.

    needs opinion regarding diagnosis and management.

    If there is tenderness over the dorsal aspect of the navicular, which is what I think you are saying, then you must consider a stress reaction or stress fracture in the navicular. An MRI scan would be the best diagnostic test for differentiaing between these two pathologic entities. Accessory navicular pathology will rarely cause tenderness dorsally over the navicular but will rather cause tenderness/pain more medially at either the joint between the body of the navicular and the accessory navicular or at the insertion of the posterior tibial tendon on the plantar-medial navicular. She should not be running if there is pain with running since, if it is a stress reaction/stress fracture, then frank fracture could occur to the navicular with "running through the pain".
