Hi all

looks like the only way forward is CSSD for instrument tracabilty, I think the time taken up with the new guide lines and documentation, will out way the cost. So would like to know of available commersial companies who will supply private practioners, and could anyone tell me of there experiences. (spell check would be helpful on this site).

4 years ago i changed over from autoclave to CSSD in my private practice.

2 years ago my NHS PCT employer changed over from autoclave to CSSD.

In my pp i had a contract with my local Acute Hospital. They were very pleased to provide the service to my pp as they made a profit from the provision of the service.

They were very relaible cost effective and certainly my pts were very impressed as i was the only pp to do this.


I was ahead of the NHS in this.

This is the way forward and this will become more attractive to PP as the new auto clave and tracability for autoclave instruments comes into being.

David how much if any tracibility are you doing, how many sets of instruments packs have you needed to go total CSSD and the cost of each pack and turn around time. I wish to do compariion trial against a totally disposable system. I would also appreciate some input from anyone actually doing the full tracibilty themselves in regard to time taken at end of sesions and what kind of record keeping they are doing.

CSSD vs Single use

My local PCT is planning on transferring to CSSD by Spring 2007. It is also trialling the use of single use instruments in the more rural areas where transport of CSSD items may be an issue.

Regardless of which method used the instruments will be fully traceable. The single use ones will be disposed of most likely by the company which supplies them. (One company definately does this, but still waiting clarification from the others)

Cost implication is huge, and difficulty with staff who may have RSI or similar. In the past this has been addressed by purchasing equipment they are comfortable with. Problem will be how to get 'their' instruments back to them.

Thank you Nikki not heard of the supplier doing the sharps disposal of their own single use. Do you know which companies these are and at what cost I wonder ?.

When your PCT moves over to CSSD will they be using their own or one of the new nucular units being built.

Legislation in regard to sterilization cross infection is changing and is continuing to change and we are being pushed towards central sterilization. To verify the changing protocol I wrote to Kevin Hall at the Society of Chiropodists and Podiatrists In which he stated “throughout the country, over the next 2- 5 years there will be a number of new super CSSD being built”, the main reason being traceability of instruments. Present recommendations to private practioners from Society of Chiropodists and Podiatrists are on achieving “an acceptable level of traceability using an autoclave with a printer”
It is therefore the responsibility of the practitioner to undertake their own risk assessment and for them to question their techniques in traceability, practicing defensively by questioning their standards of decontamination and “patient best practice”, minimizing the risks of transmission of infectious agents. Although this will increase the costs it seems the only way forward is to welcome the central sterilization as the increased time decontaminating, testing, recording and Instrument Traceability Systems will eventually out way the cost of central sterilization. But at the end of the day can we put a price on the cost of our patient’s health and safety.

From memory I think it is Teasdale surgical that will do the disposal, but will check when I am next in the office. Will be able to give a more informed answer then.

Sterile Services for Private Practioners update

I recently had a very interesting meeting with Dean Burand from IHSS sterile service, they would either pick up and deliver your own instruments for decontamination or supply you with instruments and packs of your choice on a rental scheme purchased for you to only be used by yourself. Deliveries can be once or twice a week and they would cover all tracking and tracing elements and will cover the Bucks, Herts, Beds and London areas.


Cost of example service


The cost of providing a reusable service will greatly depend on volumes, length of contract and delivery and collection schedules.

• Single Instrument £1.40 - £1.90

• Small Podiatry Pack £3.50 - £4.00

• Large Podiatry Pack £4.30 - £5.00

• Nail Surgery Trays £8.15 - £9.20
