    In-patient (hospital/wound care). Our biller billed the following:
    Procedure: 11042 Modifiers: 78 / RT
    Procedure: 97602 Modifiers: 78 / RT

    My denial states incomplete/invalid information. Would a XE modifier need to be added? The patient has been was in the hospital from 7/12/19 to 8/6/19...
