Again I have been asked to write something for this arena, I am not sure this is the right place, but I cannot find where else to put it. The issue is the phylogenetic basic for accessory naviculae. The accepted reptilian tarsus has nine bones in it, and through evolutionary time, these coalesce into the seven that we know today. The cuboid is two - tarsale 4&5, the talus is two - the tibialis and the intermedius. We see this often as the os trigonum which is an intermedius which fails to bond. However, I find myself in need of revision here. I have argued for years that while one may look to phylogeny to support an explanation for some accessory bones eg the os trigonum, one cannot for some others, eg the accessory naviculae. However, if one goes back in evolutionary time even further than the accepted reptilian tarsus, one finds a more primitive version, in which there is two centrale bones, centrale 1, and centrale 2. This morphology is only represented today in the Sphenodon (Tuatara) from some outlying islands of North Island New Zealand; it is a living fossil. That is, Centrale 1 & 2 unite - and eventually express as the navicular. Thus my comments of previous posts possibly, just possibly, may be incomplete. Although very tenuous, there is an essentially viable explanation for accesory naviculae via phylogeny. However, I still doubt this. Hope this helps. Rob

Is it just me that suspects the point may have been missed? The patient was quoted to have "flexible flatfoot and a bilateral accessory navicular". Tibialis posterior is almost guaranteed to be overworked in this situation, leading to micro damage at its main point of insertion - the navicular tuberosity. Thus a likely scenario is that the accessory naviculae were caused by the problem, and not the cause of the problem, per se. Unlike some other accessory bones one is really struggling to invoke a phylogenetic explanation for their presence as they do not (apart from a very long back in phylogenetic time) two separate bones in the reptilian tarsus. -
