Operative treatment of acute shaft and neck lesser metatarsals fractures: a systematic review of the literature

Purpose: Metatarsal fractures are relatively common injuries that they might lead to significant disability and chronic pain if suboptimally treated. Operative treatment is reserved for the displaced fractures. The primary aim of the herein study is to present the union time and rate, as well as the functional outcome of the surgically treated isolated lesser metatarsal shaft and neck fractures. The secondary aim is to present the related complications of each fixation method.

Methods: The electronic databases of Pubmed, Scopus, Embase and Cochrane libraries were searched from January 1990 to December 2020. PRISMA guidelines were used for data collection. We retrieved five articles including in total 154 patients, which were compatible to our inclusion criteria and they were used for this systematic review.

Results: A total of 75 patients were treated with percutaneous antegrade pinning resulting in AOFAS score: 96.4 ± 4.8 and time to heal 7.4 ± 1 weeks, 34 patients underwent ante/retrograde pinning resulting in AOFAS score: 95.2 ± 4.75 and time to heal 6.5 ± 1 weeks, and 45 patients underwent open reduction and internal fixation with plate and screws resulting in a time to union 10.9 ± 0.5 weeks.

Conclusion: Our results demonstrate that K wire intramedullary nailing regardless of the specific technique (antegrade, retrograde, ante/retrograde) is associated with better outcomes compared to open reduction and internal fixation as it permits faster weight bearing and quicker rehabilitation. K-wire fixation is related to statistically significant shorter time for the fracture to heal, by approximately three weeks compared to open reduction and internal fixation. Future research should focus on studies directly comparing the different intramedullary K-wiring techniques and also K-wiring versus plate fixation.

Functional outcome of 111 metatarsal fractures following conservative in comparison to operative treatment

Background: Fractures of the metatarsal bones are common injuries of the foot and particularly occur in patients aged 40-50 years. Especially multiple metatarsal fractures can lead to permanent limitations. Therefore, the aim of this study was to investigate the functional outcome of metatarsal fractures after conservative and surgical treatment using a validated self-reported patient-based outcome questionnaire.

Material and methods: All patients suffering from metatarsal fractures between 2003 and 2015 were enrolled in this retrospective analysis. The following data were collected: demographic data, AO classification, treatment, reoperation rate and the foot and ankle outcome score (FAOS). For outcome analysis, the nonparametric Mann-Whitney U‑test and Fisher's exact test were performed.

Results: In total the functional outcome of 111 patients with metatarsal fractures were analyzed, 81 patients suffered of an isolated metatarsal fracture and 30 of multiple fractures. The mean age of the patients was 45 ± 15.2 years with a total of 48 men (43%) and 63 women (57%). Patients with an isolated metatarsal fracture had an FAOS of 88 ± 17.1, while patients with multiple metatarsal fractures achieved an FAOS of 78 ± 17.7 (p = 0.046). In the group of isolated metatarsal fractures 43 patients (53%) were surgically treated and of these 36 patients showed a type C fracture (84%). In the group of multiple metatarsal fractures 16 patients (53%) underwent operative treatment.

Conclusion: Overall, the functional outcome of isolated metatarsal fractures following operative as well as conservative treatment is good to very good. Simple fractures can be successfully treated conservatively and complex multifragment fractures can be safely managed surgically. If more than one metatarsal bone is fractured, the functional outcome is significantly worse with patients reporting lasting limitations involving the range of motion and stiffness.

Fixation of displaced fifth metatarsal shaft and neck fractures

Objective: Open reduction and internal fixation of grossly dislocated fifth metatarsal shaft and neck fractures aims at restoration of the anatomical structure of the forefoot. The goal is to restore length, axis, rotation and joint position, while observing the metatarsal index (Maestro curve).

Indications: Grossly dislocated and/or open shaft/neck fractures of the fifth metatarsal; combined fractures of the forefoot involving the fifth metatarsal.

Contraindications: Lack of consent to surgery. Overall critical (life-threatening) general condition preventing surgery to the extremities. Contaminated or infected soft tissues.

Surgical technique: Depending on the planned method of fixation, open reduction is usually conducted via a lateral approach centrally above the easily palpable metatarsal V shaft. The incision lies above the glabrous skin of the sole. For markedly shortened and multifragment subcapital and shaft fractures of the fifth metatarsal, open reduction and plate fixation is the method of choice. Interlocking plates with a screw diameter of 2.0-2.4 mm are preferred to avoid later soft tissue irritation. Anatomic reconstruction is carried out under longitudinal traction at the fifth toe using small reduction clamps and, if necessary, temporary K‑wire fixation. If the fragments are large enough, one or more interfragmentary lag screws can be used for fracture compression. A straight or condylar plate is used for internal fixation. Long spiral fifth metatarsal shaft fractures may alternatively be fixed with screws. In the case of transverse or subcapital fractures, percutaneous antegrade or retrograde medullary wiring with two Kirschner wires should be considered.

Postoperative management: Following surgical treatment, rest and elevation of the injured leg, and local cooling are indicated. Subsequently, mobilization with partial weight bearing (20 kg) in foot orthosis or cast shoe for 6 weeks.

Results: Even grossly displaced fractures of the fifth metatarsal shaft have a good to excellent prognosis following surgical treatment with high union rates and rare complications. Undisplaced and mildly displaced fractures can be successfully managed nonoperatively with 6 weeks of weight bearing as tolerated in a stable orthosis or cast shoes.

Background
Various fixation methods are available for the operative treatment of metatarsal shaft fractures: Kirschner wire, Titanium elastic nail, plate, or an intramedullary bone stabilization system within a balloon catheter. The aim of this study was to compare the stability of these techniques.
Methods
72 metatarsals II to V from fresh frozen human cadaver feet were used. A shaft fracture was performed and fixed with a 1.6-mm Kirschner wire, a 1.5-mm Titanium elastic nail, a locking 6-hole-plate, or an intramedullary bone stabilization system. In a cantilever configuration, the head of the metatarsals was loaded statically (2 mm/min until failure; all groups) or cyclically (0 to 20 N for 1000 cycles with 10 mm/s, after 1000 cycles 2 mm/min until failure; plate and bone stabilization system).
Findings
The mean failure strength for static loading was 17 N for Kirschner wire, 13 N for Titanium elastic nail, 73 N for plate and 34 N for the bone stabilization system (P < .01). For cyclic loading, the mean cycle of failure was 1000 for plate and 961 for the bone stabilization system (P = .76). The mean failure strength after cyclic loading was 73 N for plate and 48 N for the bone stabilization system (P = .03).
Interpretation
Stability differs depending on the fixation method, with a plate showing the greatest stability and Kirschner wire or Titanium elastic nail the least. The stability of the bone stabilization system for fixing metatarsal shaft fractures is intermediate.

Clinical analysis of external micro-locking plate in the treatment of fifth metatarsal comminuted fractures

Objective: To explore the clinical efficacy of external micro-locking plate combined with closed reduction in the treatment of fifth metatarsal comminuted fracture with severe soft tissue injury.

Methods: From January 2018 to December 2019, 13 patients received micro-locking plate combined with closed reduction treatment due to the fifth metatarsal comminuted fracture with severe soft tissue injury. There were 11 males and 2 females patients, ranging in age from 21 to 69 years. According to the fracture AO fracture classification, 9 cases belonged to type 87(S)-C2.2 and 4 cases belonged to type 87(S)-C2.1. The fifth metatarsal cuneiform articular surface was not involved, and all of them were comminuted fractures of the fifth metatarsal shaft. All soft tissues were damaged to varying degrees. Three patients got small defect of skin and soft tissue, and they were treated with debridement and suture after one-stage emergency surgery. The fracture healing and complications were observed, and the clinical efficacy was evaluated according to the midfoot function score of American Orthopaedic Foot & Ankle Society (AOFAS) at the latest follow-up.

Results: All patients were followed up, and the duration ranged from 3 to 12 months. One patient had delayed union. One patient had local skin necrosis and was treated with second-stage skin grafting to repair the wound surface. No external screw breakage or infection was reported, the skin soft tissue healed satisfactorily and reached bony union, with the union time ranging from 8 to 19 weeks. The postoperative functional recovery was rated according to the midfoot score of AOFAS, the score ranged from 49 to 98, and 7 patients got an excellent result, 4 good, 1 fair and 1 poor.

Conclusion: The external micro-locking plate combined with closed reduction in the treatment of fifth metatarsal comminuted fracture with severe soft tissue injury has the advantages of simple operation, good stability and low infection rate, which is helpful to the repair of soft issue injury, and the fixation can be removed early in outpatient clinic. To sum up, this surgical procedure is suitable for patients with severe soft tissue injury and comminuted fracture to avoid periosteum stripping. The space occupying is smaller than the micro-external fixator, which is convenient for patients with early functional exercise.

Treatment of two or more metatarsal fractures

Introduction: Metatarsal fractures are the most common type of foot fracture. When surgical treatment is needed, pinning is typically used, either percutaneous or open. However, this fixation method has been criticized by some authors who lament residual malunion and prefer to use plate and/or screws. The primary objective of our study was to compare the outcomes of K-wire versus plate and/or screw fixation for the surgical treatment of two or more metatarsal fractures. The secondary objective was to evaluate the factors that contribute to poor outcomes and complications. We hypothesized that plate and/or screw fixation will produce better functional outcomes than K-wire fixation.

Materials and methods: This was a prospective and retrospective multicenter study carried out between 1 January 2010 and 1 June 2018 with a minimum follow-up of 12 months. Three functional scores were determined (AOFAS, FAAM and SF12 physical and mental) preoperatively, postoperatively and at the final assessment. We evaluated the outcomes in the entire study population and in four injury type subgroups, including one with isolated metatarsal fractures to control analysis bias.

Results: Our analysis compiled data from 165 patients (123 men, 42 women) who had a mean age of 38 years (16-82). The mean follow-up time was 27.9 months (10-120). There were no complications in 130 patients (79%). Skin necrosis occurred in 25 patients (15%). The FAAM score was significantly higher in the plate and/or screw group 70.2 (17-84) versus 60.3 (31-84) in the K-wire group (p = 0.033). The 78 (19-100) AOFAS was higher, but not significantly, in the plate and/or screw group versus 70 (12-100) in the K-wire group (p = 0.144).

Conclusion: Trauma to the foot that causes a fracture in two or more metatarsals often occurs due to a crush injury (39%). The frequency of associated bone lesions means that a preoperative CT scan should be done routinely to analyze the injury pattern and determine the best treatment. The fixation method should be adapted to the local conditions; when possible, it is preferable to use rigid fixation with plates and/or screws as it yields better functional outcomes.

Characterizing In-situ Metatarsal Fracture Risk During Simulated Workplace Impact Loading

Metatarsal fractures represent the most common traumatic foot injury; however, metatarsal fracture thresholds remain poorly characterized, which affects performance targets for protective footwear. This experimental study investigated impact energies, forces, and deformations to characterize metatarsal fracture risk for simulated in-situ workplace impact loading. A drop tower setup conforming to ASTM specifications for testing impact resistance of metatarsal protective footwear applied a target impact load (21-55J) to 10 cadaveric feet. Prior to impact, each foot was axially loaded through the tibia with a specimen-specific bodyweight load to replicate a natural weight-bearing stance. Successive iterations of impact tests were performed until a fracture was observed with X-ray imaging. Descriptive statistics were computed for force, deformation, and impact energy. Correlational analysis was conducted on donor age, BMI, deformation, force, and impact energy. A survival analysis was used to generate injury risk curves using impact energy and force. All 10 specimens fractured, with the second metatarsal being the most common fracture location. The mean peak energy, force, and deformation during fracture were 46.6 J, 4640 N, 28.9 mm, respectively. Survival analyses revealed a 50% fracture probability was associated with 35.8 J and 3562 N of impact. Foot deformation was not significantly correlated (p=0.47) with impact force, thus deformation is not recommended to predict metatarsal fracture risk. The results from this study can be used to improve test standards for metatarsal protection and provide performance targets for protective footwear developers.
