I am having an issue with a third party payer denying procedure code 11721 for not having the order of the dx right. I have used dx code 110.1 as primary and 250.00 as secondary those were denied. Then i sent it with 250.00 as primary dx and 110.1 as secondary dx it is still denying. Can anyone tell me what I am missing.:confused:
