    Hi,

    This patient has been referred to podiatry however has met me first! and will be waiting a couple of months for her podiatry appointment.. any thoughts welcome:

    Case study:

    HPC:
    14 yr old female.
    Saw GP with 12 month H/O gradually worsening bilateral big toe deformity
    Pain free, and wears good footwear
    No developmental problems

    O/E:
    Standing:
    Bilat great toe extention and abduction – fixed abducted position, but able to passively correct flexion
    All toes bilaterally extended and splayed
    Decreased arch
    Weight bearing through medial aspect of foot and ball of foot.

    Valgus knee posture, and hyperextended
    Tight calf

    Gait:
    Toes remain extended throughout gait cycle, and do not make contact with the floor. Achieves heel strike and pushes off from ball of foot.
