Hyperkeratoses are a common cause of foot pain due to the release of inflammatory mediators, which can have an impact on the mobility and independence of people suffering from them. However, the repercussions that hyperkeratoses have on gait parameters remain uncertain.

Aim
The aim of this study is to analyze the repercussions that plantar hyperkeratosis debridement has on several kinematic and kinetic variables of gait in a group of older participants.

Methods
98 older participants (75.1 ± 6.7 years) were randomly assigned to two groups: Group A, Scalpel debridement of plantar hyperkeratoses; and Group B, Control group (Simulated debridement). Plantar hyperkeratotic pain was measured before and after treatment on a visual analog scale. Several kinematic and kinetic variables of gait were measured before and after treatment using a Win-Track pressure sensitive walkway.

Results
A significant difference was found in the level of pain between the treated group and the control group (p < 0.01 [8.55-18.15; 95% confidence interval]). Regarding the gait parameters, statistically significant reductions were found in peak pressures (p < 0.05; Cohen’s d = 2.688) and maximum force (p < 0.04; d = 0.262).

Conclusions
Data suggests that debridement of plantar hyperkeratosis may lead to a reduction in pain and appear to reduce peak maximum force and peak plantar pressure. No significant changes were observed in the kinematic variables analyzed. The duration of the benefits remain unknown.

The Effect of Plantar Hyperkeratosis Debridement on Self-Perception of Pain Levels in older People

Background
Plantar hyperkeratotic lesions are one of the most prevalent foot problems among older people. Because of its simplicity, the most common treatment is scalpel debridement. While some studies have analysed its effectiveness in the short term or among other population groups, none has analysed its effects in the medium term. The aim of this study is to assess the effectiveness of plantar hyperkeratosis debridement compared to a control group receiving sham debridement among an older population group.

Methods
Two hundred older participants (76.4 ± 4.8 years) were randomly assigned to two groups: scalpel debridement of plantar hyperkeratoses (experimental group) or sham debridement (control group). Plantar hyperkeratotic pain was measured on a visual analogue scale.

Results
The results suggest that there were no significant differences between the groups studied just after treatment (p = 0.27), although significant differences between them were found as from 24 h after treatment (p = 0.05) and 2 (p = 0.03), 3 (p = 0.04), 4 (p = 0.04) and 5 days after treatment (p = 0.04).

Conclusions
The results of this study suggest that there were significant differences in self-perception of pain levels between the group of older people treated for plantar hyperkeratoses with scalpel debridement and the control group as from 24 h after treatment.

Foot pain and deformities are significant problems that increase with age and are significantly more prevalent in women. Calluses and corns are both common and inconvenient, and the accompanying pain complaints significantly reduce the quality of life. Professional foot care is one of the key elements in the prevention of foot disorders and improves the comfort of people with such problems. The aim of the present research was to assess the impact of podiatry treatments on the foot conditions, including parameters related to foot geometry, the forefoot pressure on the ground, and the occurrence of pain in women over 50. In each volunteer, history and physical examination were performed and an objective assessment with the use of the digital foot scanner and the baropodometric measurements was performed. The examinations were conducted before the beginning of the procedures and up to a week after each of the three podiatry treatments performed at monthly intervals. Specialist foot care treatments influenced the values of the hallux valgus angle, the varus angle of the fifth toe, and the longitudinal arch. After each treatment, the percentage of feet with normal longitudinal arch was significantly greater than before the treatments. The treatments influenced the value of pressure in the forefoot. The percentage of feet with pain decreased significantly after each treatment. Performed podiatry treatments, consisting of the removal of changes related to keratosis, not only normalized the parameters related to foot geometry and the forefoot pressure on the ground but also reduced foot pain.
