Background:Higher plantar pressures at the medial forefoot are reported in hallux valgus. Foot orthoses with medial arch support are considered as an intervention in this pathology. However, little is known about the effect of foot orthoses on plantar pressure distribution in hallux valgus.

Objectives:To investigate the effect of a foot orthosis with medial arch support on pressure distribution in females with mild-to-moderate hallux valgus.Study design:Quasi-experimental.

Methods:Sixteen female volunteers with mild-to-moderate hallux valgus participated in this study and used a medial arch support foot orthosis for 4 weeks. Plantar pressure for each participant was assessed using the Pedar-X® in-shoe system in four conditions including shoe-only and foot orthosis before and after the intervention.

Results:The use of the foot orthosis for 1 month led to a decrease in peak pressure and maximum force under the hallux, first metatarsal, and metatarsals 3-5 (p < 0.05). In the medial midfoot region, peak pressure, maximum force, and contact area were significantly higher with the foot orthosis than shoe-only before and after the intervention (p = 0.00).

Conclusion:A foot orthosis with medial arch support could reduce pressure beneath the hallux and the first metatarsal head by transferring the load to the other regions. It would appear that this type of foot orthosis can be an effective method of intervention in this pathology.

Clinical relevance:Findings of this study will improve the clinical knowledge about the effect of the medial arch support foot orthosis used on plantar pressure distribution in hallux valgus pathology.

Objective: Hallux Valgus is one of the most foot deformities which increases plantar pressure beneath big toe and first metatarsal. The aim of this study was to assess the effect of foot orthosis on plantar pressure distribution in subjects with mild and moderate Hallux Valgus.
Materials & Methods: in this quasi-experimental study, females 16 with Hallux Valgus were recruited. Plantar pressure in eight areas of foot was measured by Pedar-X insole when wearing standard shoe only; shoe with foot orthosis and shoe with foot orthosis after one month of using orthosis. Data were analyzed by repeated measure analysis of variance test.
Results: using foot orthosis for a month leaded to decrease pressure in the big toe (p<0.019); first metatarsal and 3-5 metatarsals (p<0.001) and also increased pressure in medial mid foot (p<0.001).
Conclusion: Foot orthosis decreased peak pressure in fore-foot and increased it in medial mid-foot. Therefore redistribute plantar pressure to the more normal pattern in Hallux Valgus subjects. So it could be one of the effective methods to prevent the progression of this deformity in its initial steps of formation.

Influence of hallux valgus deformity on forefoot pressure distribution of Chinese diabetic patients

Since issues about hallux valgus deformity (HV) in diabetic population were rarely reported, this study was aimed at investigating the influence of HV on their forefoot pressure distribution. Twenty-seven diabetic patients with HV (DMHV) and 27 diabetic control ones without HV (DMC) were recruited. Patients were inspected by an experience clinical expert and those with other deformities, a history of ulceration, neuropathy, or amputation were excluded. Barefoot plantar pressures while walking were measured by Emed-at pressure plate in their selected speed and in a protocol of two-step initial. Variables of relative centre of pressure oscillation in medial-lateral direction (%) (COPxrel), COP velocity (m/s) (COPv), peak pressure (kPa) (PP), relative maximum force (%) (MFrel), relative contact area (%) (CArel), and relative pressure/force-time integrals (%) (PTIrel/FTIrel) were calculated for the area of the hallux and the medial, central, and lateral forefoot. Results of independent t test with a significant level of 0.05 and 95 % confidence interval show that MFrel and CArel of DMHV at hallux were recorded significantly lower than those in DMC by 38.48 % (P = 0.02) and 2.86 % (P = 0.001), while CArel, FTIrel, and PTIrel of DMHV beneath medial forefoot were shown significantly larger than those DMC by 3.99 % (P = 0.002), 31.52 % (P = 0.04), and 7.14 % (P = 0.02). No significant differences between DMHV and DMC were found for variables of COPxrel (P = 0.60) and COPv (P = 0.39), as well as for force load transfer at the forefoot area. Overall, DMHV is featured with pressure off-load under the hallux and pressure over-load at the medial forefoot; moreover, forefoot stability and its normal load transfer mechanism would not be affected by HV when the angle of hallux is less than 25°.

ANALYSIS OF FORCE DISTRIBUTION PATTERN UNDER THE FORE-FOOT IN PATIENTS WITH HALLUX VALGUS DEFORMITY

The purpose of this paper is to study the force pattern distribution under the fore-foot areas with and without Hallux Valgus deformity. Hallux Valgus (HV) is one of the most common foot deformities. Plantar force pattern in HV patients has been widely studied in previous published papers to identify HV condition according to their plantar force pattern. This condition occurs by the ratio of two to one women to men. People with HV have altered pressure pattern. As 23% of people develop such condition, studying and understanding the plantar force pattern is essential. Furthermore, speed of walking has an effect on force changes under the foot. So in order to examine the effect of different walking speeds on plantar force pattern, twenty people ten with and ten without HV took part in the test by walking on the pressure mat. The results showed that the variability in force pattern in the same individual was significant from one trial to another which can be concluded that obtaining consistent force pattern is an issue to overcome. After getting consistent force readings, it was observed that the higher load was under the 2nd and 3rd metatarsal heads in people with HV which was significantly different from non-HV participants. There are contradictory opinions in previous published papers on plantar pressure pattern as a means of identifying HV. Furthermore, analysing force pattern is a hurdle to overcome. As walking speed has significant effect on plantar pressure patterns, the study of this issue is badly needed.
