Can the development of peripheral artery disease (PAD) in the lower limb be in any way slowed or otherwise beneficially affected by restoring the mass of intrinsic foot musculature lost through diabetic myopathy (atrophy ) ?
In my opinion ,possibly .
Briefly , my understanding of the disease process is that blood flow to the foot is increased in the early stages of the disease but that the blood is rapidly return to the venous system via arterio venous shunting .This is accompanied by hardening of the arteries ( arteriosclerosis ) and eventually occlusive artheroschlerosis develops .
Perhaps if the intrinsic muscle tissue of the foot were preserved by resistance exercise , then the pulsatile nature of blood flow through the arteries would could be maintained , providing the shear forces necessary for a greater degree of health and proper functioning of the endothelial cells .
Of course the pathogenesis of PAD is multifactorial but the retention of the vascular muscle beds of the foot may be off benefit .
