I think I have a stupid question but let me to try to find an answer....
Do you know any instrument in the form of a phone interview focused on foot orthotic services addressed to the patients who is calling at front desk for an apointment, in order to have some kind of gross/first classification (or an initial score) of the complexity of their feet problems?

Hope I've expressed enough clear my problem!
