The medial tibial stress syndrome score: a new patient-reported outcome measure

Background At present, there is no validated patient-reported outcome measure (PROM) for patients with medial tibial stress syndrome (MTSS).

Aim Our aim was to select and validate previously generated items and create a valid, reliable and responsive PROM for patients with MTSS: the MTSS score.

Methods A prospective cohort study was performed in multiple sports medicine, physiotherapy and military facilities in the Netherlands. Participants with MTSS filled out the previously generated items for the MTSS score on 3 occasions. From previously generated items, we selected the best items. We assessed the MTSS score for its validity, reliability and responsiveness.

Results The MTSS score was filled out by 133 participants with MTSS. Factor analysis showed the MTSS score to exhibit a single-factor structure with acceptable internal consistency (α=0.58) and good test?retest reliability (intraclass correlation coefficient=0.81). The MTSS score ranges from 0 to 10 points. The smallest detectable change in our sample was 0.69 at the group level and 4.80 at the individual level. Construct validity analysis showed significant moderate-to-large correlations (r=0.34?0.52, p<0.01). Responsiveness of the MTSS score was confirmed by a significant relation with the global perceived effect scale (β=−0.288, R2=0.21, p<0.001).

Conclusions The MTSS score is a valid, reliable and responsive PROM to measure the severity of MTSS. It is designed to evaluate treatment outcomes in clinical studies.

Background: The incidence of adolescent overuse injuries, including bone stress injuries (BSIs), is on the rise. The identification of
a BSI in the early stages is key to successful treatment. The Shin Pain Scoring System (SPSS) was developed to aid clinicians in
identifying patients with a BSI.
Hypothesis: The SPSS will correlate with magnetic resonance imaging (MRI) grading of a BSI in an adolescent population.
Study Design: Cohort study (diagnosis); Level of evidence, 2.
Methods: Enrolled in this study were 80 adolescent high school athletes between the ages of 13 and 18 years participating in a
variety of sports with more than 1 week of atraumatic shin pain. The SPSS questionnaire was completed for each participant, and
physical examination findings were recorded. Each question and physical examination item was allotted a point value, which
totaled 29 points. Radiographs and MRI scans of both lower legs were obtained for each participant. The SPSS score was statistically analyzed using logistic regression, a classification matrix, and a 2 2 contingency table to evaluate validity and
predictability.
Results: Logistic regression analysis of our data determined that 3 categories of SPSS scores provided the highest diagnostic
value when compared with MRI grading based on the Fredericson classification (0-4). The SPSS correctly identified 43.5% of
injuries for category 1 (MRI grades 0-1), 62.5% for category 2 (MRI grade 2), and 50.0% for category 3 (MRI grades 3-4). Overall,
the SPSS correctly identified the degree of BSI in 54.4% of all tibias studied. Binary analysis for validity demonstrated a sensitivity
of 96%, specificity of 26%, positive predictive value of 76%, and negative predictive value of 71% for the SPSS relative to the “gold
standard” MRI results.
Conclusion: The SPSS is a potentially valid method to identify tibial BSIs, given the sensitivity and negative and positive predictive
values. It also provides helpful categorization to alert clinicians to the presence of a BSI and direct further diagnostics and/or
interventions. The SPSS should be considered as an additional tool to use when evaluating adolescents with atraumatic tibial BSIs.

Systematic Review of Patient-Reported Outcome Measures for Patients with Exercise-Induced Leg Pain

Background and Objectives: To determine the most commonly used patient-reported outcome measures (PROMs) in exercise-induced leg pain (EILP) and to identify specific PROMs for EILP in order to evaluate their psychometric properties and methodological quality. Materials and Methods: A strategic search was performed in different databases to identify and extract the characteristics of studies based on the use of PROMs in patients with EILP. Specific PROMs were evaluated according to the Terwee et al. and COSMIN criteria. Results: Fifty-six studies were included in the review. The Medial Tibial Stress Syndrome Score (MTSSS), Lower Extremity Functional Scale (LEFS) and Exercise-Induced Leg Pain Questionnaire (EILP-Q) were identified as specific PROMs for EILP. The Visual Analog Scale (VAS) was the most widely used instrument in the assessment of EILP. The methodological quality assessment showed six positive values for the LEFS, four for the MTSSS and three for the EILP-Q for the eight psychometric properties analyzed according to the COSMIN criteria. The evaluation of the nine psychometric properties according to Terwee showed five positive values for the LEFS and MTSSS, and three for the EILP-Q. Conclusions: The overall methodological quality of the PROMs used was low. The VAS was the most widely used instrument in the assessment of EILP, and the LEFS was the highest quality PROM available for EILP, followed by the MTSSS and EILP-Q, respectively.
