Objective
To evaluate the reliability and used diagnostic criteria and pressure pain thresholds and the prevalence of LTrPs in several muscles of the lower limb in subjects with a MLA compared with controls.

Design
Cross sectional study. An internal pilot study was performed to calculate the sample size (precision level of 5% and 80% power) and to study the intrarrater reliability.

Setting
University campus.

Participants
Eighty two lower MLA subjects and eighty two controls.

Intervention
No applicable.

Main outcome measures
The NDT was used to classified lower MLA subjects (≥ 10 mm) and controls (5-9 mm). The Travell and Simons specific diagnosis criteria and pressure pain threshold were used to evaluate the prevalence of the LTrPs located in several muscles of the lower limb and compared between groups. The reliability was evaluated using Cohen´s kappa and ICC. The unpaired Student t test and the X2 test were used to evaluate the difference in the LTrP prevalence between groups.

Results
The intrarrater reliability was excellent for the NDT and the diagnosis of LTrPs, being the taut band and the spot tenderness the most reliability specific diagnosis criteria. In the lower MLA group, sixty subjects (73%) presented at least one LTrP, whereas fifty seven controls (70%) presented at least one. The lower MLA group showed more LTrPs (4.46±4.10) than controls (3.32±3.24) (p<0.05). There were significantly (p<0.05) more subjects with LTrPs in FDL, TA and VM in the lower MLA group compared with the control group.

Conclusions
LTrPs are common in lower limb muscles, both controls and lower MLA subjects. A lower MLA is associated with a greater prevalence of LTrPs, being significant in FDL, TA and VM.

This mixed-method blinded research study used high-frequency diagnostic medical sonography (DMS) to document myofascial trigger points (MTrPs) associated with ankle/foot pain. A total of 17 symptomatic and 8 asymptomatic participants provided 500 MTrP areas for palpation. Forty-nine of these MTrP areas (including 16 tender points, 15 palpable MTrPs, and 18 palpated and imaged MTrPs) were compared with the patient’s ankle/foot pain, collected with the SF-12 Health Survey, Victorian Institute of Sports Assessment–Achilles questionnaire, and a visual analog scale. Qualitative analyses of the participants’ histories were used to understand the context of the data collected. The mean area of the 18 imaged MTrPs was 0.09 cm2, and they appeared inhomogeneous and hypoechoic. Those with right-sided MTrPs were statistically significant for those with reported pain. Participants with left-sided MTrPs did not demonstrate a significant correlation to reported pain. The results demonstrated a promising possible diagnostic approach using sonography to evaluate MTrPs in the ankle/foot for documenting myofascial pain. In addition, elastography and transducer frequencies >12 MHz are proposed as a DMS technique to increase overall diagnostic yield.

The influence of myofascial release on pain and selected indicators of flat foot in adults: a controlled randomized trial

Flat foot pain is a common complaint that requires therapeutic intervention. Currently, myofascial release techniques are often used in the therapy of musculoskeletal disorders. A group of 60 people suffering from flat feet with associated pain. Patients were assigned to four groups (15 people each): MF-myofascial release, E-the exercise program, MFE-myofascial release and the exercise program, C-no intervention. The rehabilitation program lasted 4 weeks. The NRS scale was used to examine pain intensity and FreeMed ground reaction force platform was used to examine selected static and dynamic foot indicators. Statistically significant pain reduction was obtained in all research. A static test of foot load distribution produced statistically significant changes only for selected indicators. In the dynamic test, statistically significant changes were observed for selected indicators, only in the groups subjected to therapeutic intervention. Most such changes were observed in the MF group. In the dynamic test which assessed the support phase of the foot, statistically significant changes were observed only for selected subphases. Most such changes were observed in the MFE group. Both exercise and exercise combined with myofascial release techniques, and especially myofascial release techniques alone, significantly reduce pain in a flat foot. This study shows a limited influence of both exercises and myofascial release techniques on selected static and dynamic indicators of a flat foot.
