    I am due to see a 22 month old child whose mother is concerned about his foot alignment. I have never treated a child this young and my inclination is to give advice on supportive footwear but otherwise adopt a wait and see approach. Based on the attached photo would anyone recommend orthotic treatment in a child this young?

    Agree with CP, be cautious. Maybe simple heel wedging at this stage and review 6 monthly.

    Thank you. Absolutely agree.
    I explained to the Mum that a flat foot (ie no medial arch) is normal at this age but, having seen the photo, it is clear that there is quite pronounced pronation, even for a child this young.
