27 year old F with sudden onset of pain at R 4th plantar met head- pain was felt on placing foot on floor, no injury, trauma etc. Pain throbbing and felt walking painful. Unable to dorsiflex 2-4 toes. Took 2x 500mg paracetamol and later in evening 3x 200mg Ibuprofen and got colleague to look at it following day.
O/E: Swelling and bruising over R4th met head and very tender to palpation. Unable to passively dorsiflex 2-4 toes. Limited ROM at B1st. Limited DF at R ankle. Unable to DF ankle against resistance. No sensory loss and able to differentiate between sharp and blunt. Got an x-ray to rule out stress fracture and didnt feel that there was an indication of a mortons neuroma.
Colleague made offloading insole and have been icing area regularly and continuing with ibuprofen- swelling and pain 50% improved but i still cant move my lesser toes 6 days on.
I do have a prolapsed disc at L5 level and sciatica associated with this. Otherwise GGH and no medication (Amitripyline and Gabapentin dont "agree with me" so i just get on with the sciatic pain). Feel a bit embarrased asking this question as I know I should be able to work it out but just cant quite understand it so any guidance would be very much appreciated.
Thanks in advance
Linda
